The incumbent serves as Liaison for the Contract Nursing Home (CNH) program. The incumbent provides direct patient care services. Responsibilities include the management, coordination and provision of social work services to veterans and their families in community settings. The incumbent will participate in regular multidisciplinary inspections of CRCP and CNH homes to ensure that veterans placed in these settings are receiving appropriate psychosocial services.
Work Schedule: Full-Time, Monday to Friday, 6:00 a to 6:00 pm (per supervisors approval)

The incumbent independently conducts psychosocial assessments to determine the needs of veterans and family members. He/She is an active participant in the treatment planning process and works in collaboration with the patient/family, the VA and CNH interdisciplinary treatment teams. The incumbent is able to coordinate community based services, including information and referral for additional services from other VA government, and community agency programs. The incumbent assists with discharge planning from the Contract Nursing Home and coordinates with the nursing home, PACT and/or specialty programs to ensure continuity of care. The incumbent independently provides consultation services to other staff regarding the psychosocial needs of veterans and the impact of psychosocial problem on health care and adherence to treatment plans. The incumbent will develop an assessment of the veteran in collaboration with interdisciplinary treatment teams, the veteran, family members and significant others, whenever possible. The goal of the assessment is to highlight the veteran's strengths, limitations, and internal/external supports and service needs in order to optimize the veteran's functional status and safely maintain the veteran in the least restrictive environment. The incumbent will, as appropriate, supervise the practice of social work students, or a social working operating without an independent license. The incumbent will participate in committee assignments as deemed necessary by the service manager. The incumbent is responsible for assuring that monthly follow-up visits to veterans in the CNHs are completed and documented utilizing CPRS and ensure appropriate hand off when transferring of patient care, as required by the program. The incumbent is responsible for providing input to the CNH Coordinator and/or Supervisor to assist in assuring that Quality Assurance monitors related to these programs are completed and reported to the CNH Oversight committee. In consultation with the CNH Coordinator and/or Supervisor, the incumbent is responsible for assisting with reporting and investigating Sentinel/Adverse events to the local VA, VISN, and Central Office. The incumbent is involved in the annual inspection of the nursing homes with lead support provided by the CNH Coordinator and/or Supervisor. The incumbent performs other duties as assigned.
Physical Requirements:
The work is moderately active. Typically, the employee is driving a vehicle, walking around various facilities/settings, visiting with patients standing up, and at times sits comfortably while working. There will also be some light exertion required when walking; standing, bending or climbing stairs, carrying of light items such as papers, books, laptop, small objects, or driving an automobile. Must be able to sit/stand for long periods of time and lift 20lbs. Ability to complete a Basic Life Support (BLS) course is a position requirement.
