The PACT Physician Assistant in Primary Care - Patient Aligned Care Team (PACT) team is accountable for adhering to established policies and standards of care/standards of practice in evaluating, tracking, and following up on patient care. The incumbent works in collaboration with health care professionals to improve the quality of care in VA Central Western Massachusetts Healthcare System.
Work Schedule: Mon - Fri, 8:00 am - 4:30 pm

Education Debt Reduction Program (EDRP): Authorized
This position is eligible for the Education Debt Reduction Program (EDRP), a student loan reimbursement program within VHA. If selected for this position, you are encouraged to apply for this award program and must meet specific individual eligibility criteria in accordance with VHA Handbook 1021. Employees must apply for EDRP within four (4) months of appointment. Final award amount and program start date is determined after review of a complete EDRP application and supporting documentation by the VHA Education Loan Repayment Services team. Participation in EDRP is contingent on the availability of EDRP funding and is not guaranteed.
Routine Duties may include (both for Nurse Practitioner and Physician Assistant):
(1) Performing initial medical histories and physical examinations.
(2) Providing and coordinating comprehensive care for assigned patients.
(3) Managing medical problems to include writing orders for medications for stable medical conditions and treating minor illness; ordering laboratory tests; ordering imaging tests; adhering diets and over-the-counter medications and supplies; reviewing diagnostic tests; reports and consults.
(4) Observing and recording patient's medical progress, updating and summarizing medical records, and changing orders when appropriate. Notifying the responsible Physician of significant changes in the patient's condition, in such cases there must be documentation of consultation with the supervising Physician. 'Notes and Orders' do not routinely have to be co-signed. The Nurse Practitioner/Physician Assistant may be delegated to document the discharge summary, but the supervising Physician must write the discharge note or co-sign the discharge summary.

Physical Requirements: Position is subject to a pre-employment physical and medical examination. This position requires: Light lifting, under 25 pounds; light carrying, under 25 pounds; reaching above shoulder; use of fingers; walking (8 hours); standing (8 hours); repeated bending (1 hour working closely with others; working alone; protracted or irregular hours of work and occasional emotional strain.)
