Nurse Practitioner - Community Living Center
The Nurse Practitioner in Extended Care and Rehabilitation is assigned to provide care to Veterans in the Community Living Center (CLC). These duties may include providing care to Veterans on our Memory Care Unit or patients admitted for skilled nursing care or acute rehabilitation. The Nurse Practitioner works collaborative as part of an inter-disciplinary team with geriatric psychiatry, nursing, therapists, psychologists, social workers, dieticians, pharmacists.
