The South Texas Veterans Health Care System (STVHCS) is comprised of two inpatient campuses: the Audie L. Murphy Memorial VA Hospital in San Antonio and the Kerrville VA Hospital in Kerrville, Texas. The STVHCS serves one of the largest primary service areas in the nation, with 63 counties, and is part of the VA Heart of Texas Veterans Integrated Service Network (VISN 17), with offices located in Arlington, Texas. Providing a service to Veterans with a working staff of more than 3,100 employees, South Texas provides health care services for 93,000 Veterans. The STVHCS is a facility providing primary, secondary and tertiary health care in medicine, surgery, psychiatry, and rehabilitation medicine. It also supports a 90 bed Extended Care Therapy Center, a 30-bed Spinal Cord Injury Center, an eight-bed Bone Marrow Transplant Unit, and a Geriatric Research, Education and Clinical Center. Affiliated with the University of Texas Health Science Center at San Antonio, the system has an active ambulatory care program with numerous satellite outpatient clinics.

The RN Care Navigator plays a key role in the management of patient care and ensures patients are tracked and followed and serves as a point of contact for the subspecialty. The Care Navigator works under the direction of the Service Chief, receives specific direction from Section Chiefs, and works collaboratively with providers and clinicians across the health care system. This Nurse is a licensed registered nurse responsible for identifying and facilitating options and services for meeting patients' health needs while decreasing fragmentation and duplication of care and enhancing quality, cost-effective clinical outcomes. She/he collaborates with internal interdisciplinary team members to initiate referrals, access resources and to problem-solve clinical and/or social issues, related to care plan development and/or implementation. The Navigator also interacts with external fee providers and agencies, Department of Defense partners, and other community providers and agencies to identify and coordinate services not available through South Texas Veterans Health Care System.
The Care Navigator reports directly to the Chief of Surgical Service who in turn reports to the Chief of Staff with a matrix to the Deputy Associate Director for Patient Care Services (Deputy Nurse Executive) for nursing practice oversight.
Additional Performance Standards:
Advises Section Chief and Surgical Service leadership on clinical and administrative issues related to subspecialty care delivery and access.
Promotes and utilizes evidence based guidelines at all stages of subspecialty care access. Develops and maintains content expertise in clinical care through peer review, networking, published literature, national conference calls and educational offerings and professional meetings
Contributes to system improvement projects regarding access to subspecialty care.
Participates in strategic planning, business plan development, implementation, monitoring, and evaluation of the subspecialty programs. Complies with all VA and STVHCS Policies, especially those related to consult management and appointment scheduling.
Identifies patient population and individual patients in need of case management services for ENT, Ophthalmology and other surgical specialties as needed;
and other duties covered by the Registered Nurse (Surgery Care Navigator) functional statement.

Annual leave shall accrue for full-time Nurses, and Nurse Anesthetists at the rate of 8 hours for each full biweekly pay period.
