The Geriatrics and Extended Care (GEC) Manager and Palliative Care Champion, referred to hereafter as GEC Manager. The incumbent is a professional nurse accountable for primary assistance to the VISN 21 Chief Medical Officer for delivery of clinical care and the oversight of geriatrics personnel and resources within the VISN. The incumbent is also responsible for coordination of performance improvement, utilization review, risk management, and external review activities at VISN 21 Geriatrics and Extended Care facilities, including community living facilities (also known as NHCUs), community nursing homes and state veterans' homes. Further, the incumbent serves as the VISN Clinical Champion for Palliative Care.
The GEC Manager: Provides executive leadership, oversight and facilitates overall performance improvement activities within the VISN 21 GEC programs. Works towards achievement of high quality, safety, and cost effective evidence based outcomes on a continual journey of a high reliability organization. Collaborates with both administrative and clinical services to monitor quality and safety throughout the VISN 21 GEC programs. Functions as facilitator in implementing action plans and process improvement strategies at the VISN and medical facilities for follow up to include regular progress reports. Reviews Performance Measures, EPRP, VASQIP data, IPEC, and other quality data for trends and early intervention. Applies clinical performance improvement methods, collects outcome data related to policy, procedure, and regulatory compliance and in collaboration with management, initiates performance improvement monitoring projects. Utilizes data and comparative analytical processes in recommending design and implementation of innovative programs to improve service excellence, customer satisfaction, and compliance to patient care and regulatory standards.
