The clinical pharmacy specialist (CPS) functions at the highest level of clinical practice, works independently under their scope of practice as defined by the individual medical center to directly care for patients. A CPS plays a defined role in budgetary execution and serves as a mid-level provider who functions to initiate, modify or discontinue medication therapy and as a consultant for intensive medication therapy management services. This includes, but is not limited to, the following: designing, implementing, assessing, monitoring and documenting therapeutic plans utilizing the most effective, least toxic and most economical medication treatments; helping achieve positive patient centric outcomes through direct and indirect interactions with patients, providers, and interdisciplinary teams in assigned areas; performing physical assessments; and ordering laboratory and other tests to help determine efficacy and toxicity of medication therapy. The CPS is a clinical expert and leader within the Pharmacy Department. As an assigned service line team member, the incumbent functions under the matrix supervision of the Chief of Pharmacy and the Associate Chief of Clinical Pharmacy Services, while directly reporting to their respective clinical pharmacy supervisor.
Work Schedule: Monday - Friday, 8:00 AM - 4:30 PM
This medical center operates 24 hours, seven days per week and you may be required to work days, evenings, nights, holidays, and/or weekends, to include modification in shifts and/or tours of duty, to meet medical center and patient care needs.

The CPS shall routinely maintain an outstanding and up-to-date clinical knowledge base about medication therapy, contemporary journal articles, and contemporary pharmacy practice. Incumbent will routinely provide and support leadership and management oversight as it relates to the clinical programs and clinical activities of assigned personnel. Responsibilities shall include:
(1) Scanning the internal and external environments to determine opportunities to enhance pharmacy services to support patient education, patient adherence, clinical outcomes, and team member growth and development.
(2) Monitor drug utilization (formulary, non-formulary, restricted) and intervene to ensure formulary adherence and optimal evidence-based clinical outcomes.
(3) Monitor the educational needs of providers, pharmacists and patients, and make recommendations to address any deficiencies. Actively participate in supporting any defined educational programs and activities.
(4) Routinely benchmark with national contemporary programs and make recommendations for clinical pharmacy service and lead program changes when directly involved in the assigned practice area.
(5) Analyze and interpret the published literature, and appropriately apply one's findings. In addition, demonstrate a willingness to maintain a current understanding in terms of the breadth and depth of the literature in appropriate clinical areas.
(6) Routine review of medical orders for appropriateness of the drug, dosage, schedule, duration, and directions.
(7) Objectively support adherence to the formulary and demonstrate the assertiveness and poise that is needed to handle any untoward situations. Support the review and adjudication of non-formulary medication orders consistent with departmental guidelines / policy.
(8) Routine support for medication formulary compliance, medication conversions, therapy treatment and pathway compliance, and drug utilization evaluations as assigned.
(9) Routinely provide medication information to providers, assigned patients, and members of the clinical pharmacy team. This activity is both formal and informal and may involve both written and oral communication. Medication information consultations generally cover aspects of pharmacotherapy, including the rational and appropriate use of drugs, pharmacokinetics, pharmacology, toxicology, and the management of drug and chemical toxicities.
