This position is physically located at the VA Community Based Outpatient Clinic (CBOC) in Oceanside, CA.
The Senior Social Worker practices in the Transition Care Management Program and serves a patient population with highly complex health and mental health problems. The Senior Social Worker is assigned administrative responsibility for clinical program development and is accountable for clinical program effectiveness and modification of service patterns. The Senior Social Worker serves as Point of Contact (POC) between the DOD and VA for returning veterans and assisting with the transition to VA Healthcare. This includes independent case management, clinical treatment, advocacy, and coordination of linkages to appropriate VA or community service providers / agencies as needed by the client. This is accomplished in cooperation with other members of the interdisciplinary treatment team including the OEF/OIF PTSD Clinic and Mental Health Clinic.

The Senior Social Worker performs specialized treatment of complex physical and mental illness for patients who are OEF/OIF combat veterans and veterans who were medically separated from active duty via Medical Evaluation Board (MEB)/ Physical Evaluation Board (PEB), incorporating complex multiple causation in differential diagnosis and treatment of veteran patients, including making psychosocial and psychiatric diagnoses within approved clinical privileges or scope of practice.
Specific tasks may include:
> Providing long term case management and care coordination,
> completion of biopsychosocial assessments,
> treatment planning/goal setting,
> referral(s) to service providers and specialty treatment clinics,
> resource utilizations and development,
> advocacy, crisis intervention,
> education and prevention,
> supportive counseling,
> tracking of veterans through various systems/databases,
> outreach at functions including but not limited to Post-Depolyment Health Reassessments (PDHRA), Yellow Ribbon Reintegration Program (YRRP) and Demobilizations
> individual, group, and family psychotherapy and advanced level case management interventions used in the treatment of veterans with serious medical and psychiatric disorders.
> continuum of care asssments.
The Social Worker:
> provides services on an outpatient basis
> develops and implements methods for measuring effectiveness of social work practice and services in the Transition Care Management Program specialty area, utilizing outcome evaluations to improve treatment services.
> designs changes based on empirical findings.
> offers consultation to colleagues and students on the psychosocial treatment of patients treated in specialty areas, rendering professional opinions based on experience and expertise and role modeling effective social work practice skills.
> teaches and mentors staff and students in the special area of practice
> provides supervision for licensure of specialty certifications
