The Maternity Care Coordinator (MCC)/RN Care Manager demonstrates advanced clinical knowledge, leadership and mastery of nursing practice in delivering efficient, comprehensive and continuous patient-driven holistic care through active collaboration, communication, and coordination of resources for women Veterans. As a change agent, the MCC promotes an environment that embraces new knowledge, innovations and improvements. The MCC is responsible for ensuring that there is a mechanism in place that addresses the following elements: monitoring the delivery of services, coordination of care, and tracking of maternal and fetal outcomes. This includes ensuring coordination of non-VA maternity care with relevant VA providers, especially if the Veteran has pre-existing medical or mental health conditions. The MCC will coordinate and monitor the delivery of maternity care, act as a as a liaison between the patient, the non-VA provider, and the facility. Provides leadership in implementation and evaluation of guidelines, professional standards and policies that impact women Veterans. Develops and promotes systems to improve access and continuity of care. Uses advanced clinical knowledge and critical thinking skills to mentor staff in planning, implementing and evaluating interventions that improve patient outcomes. The MCC uses clinical knowledge and skill and a variety of information technologies including clinical databases, to aid in risk assessments and other surveillance strategies to customize interventions and promote achievement of health related outcomes and patient safety. Designs and provides age and population specific health promotion and risk reduction strategies. Translates evidence-based research into practice to ensure that patients benefit from the latest innovations in nursing science. Serves as a resource and advocate in the protection of patient and family rights. Effects change through advocacy for the profession, interdisciplinary health care team and the Veterans.
