The geriatrician will provide clinical services in the Leavenworth Community Living Center, and will serve as the Medical Director for this unit. It provides long term care, post-acute care, and hospice care beds.

This position will also cover intermittently for other Providers (physicians, nurse practitioners, and Physician Assistants) on an as-needed, intermittent basis within the Geriatrics and Extended Care Service Line, both in Leavenworth and Topeka. These services include other Community Living Center units, Geri-PACT (geriatrics primary care), Geriatrics Ambulatory consultation clinics, palliative care consultation team, and for the Home-based Primary Care physician, or other geriatrics service needs.
The geriatrician will perform a variety of tasks including triage, medical assessment, medication reconciliations, history and physical examinations, medical decision-making, ordering and interpreting diagnostic tests as needed, and managing medical treatment plans. This position will also participate in developing interdisciplinary treatment plans and attend team meetings.
The geriatrician will perform as part of a multidisciplinary treatment team that includes a nurse
practitioner, nursing staff, pharmacists, dietitians, psychologists and social workers. Similar teams exist in other areas of the service line in which the incumbent may provide occasional intermittent coverage.
Medical Direction activities are also an important part of this position, working closely with nursing management and other personnel to guide admission and discharge policies, lead and participate in quality improvement and patient safety activities, serving on related committees and leading committees as appropriate, working to support compliance with policy and procedures, participating in national and regional medical director calls and tasks, and participating in continuing education for CLC employees.
This position may also participate in the education of learners of VA affiliates from a range of health professional programs, many of whom require geriatrics-specific experience. This position will also participate in facility-level committees, QI initiatives, and educational activities.
