This position is located within the Medicine Service of the Denver VA Medical Center in Denver, Colorado. The position is aligned under the Division of Endocrinology and Diabetes and reports directly to the Chief of Endocrinology, Diabetes and Metabolism Section and Chief, Medical Service. This position has a collaborative relationship with all specialty care services within the network.
The Endocrinology Clinical Diabetic Educator (CDE) practices in the inpatient and outpatient settings at the Denver VA Medical Center and helps coordinate Endocrinology Diabetes education within the VISN. The CDE is academically and clinically prepared to perform a broad spectrum of patient and staff diabetic education at the expert level.
The position applies in depth knowledge and skills in the provision of diabetes self-management education and ongoing support to those affected by diabetes. Working in a collaborative practice setting, the CDE or BC-ADM will work closely with Endocrine faculty, advanced nurse practitioners, and program support staff as well as individual veterans in the outpatient and inpatient setting.
The CDE or BC-ADM is responsible for creating, delivering, scheduling, and planning a diabetes educational series for programs such as SCAN ECHO and PEERs. These type of series support the professional development of diabetes educators and other healthcare professionals including nurses, dieticians, pharmacists, primary care providers locally and nationally.
The Certified Diabetes Education is an experienced health care professional with a strong background in diabetes education and willingness to commit to Endocrine management of patients, families and provider education. The CDE is academically and clinically prepared to perform a broad spectrum of patient/provider education at the expert level. The position will incorporate in person and remote education through direct patient care as well as case discussion to nurses, pharmacists and local and remote primary providers via TELE Endo and the the Glucose Education team (GET) consults inpatient, outpatient diabetes management. The CDE will help the NPs with E-consults on endocrine disorders and diabetes management. The CDE will work closely with Endocrine faculty, Advanced Nurse Practitioners, Endo CDE and program support administrators to ensure implementation and optimization of inpatient and outpatient diabetes education and care as well as educational programs. In addition, the CDE will have responsibility to provide educational resources (provider and patient) for the Transition of Care Program for veterans with diabetes leaving the hospital. The CDE is responsible for coordinating, scheduling, and planning educational series based on requests from providers.
Certification by the National Certification Board for Diabetes Educators is required.
