This position is located at the Jamaica Plain, MA Campus of VA Boston
This is a Nurse Manager (NM) position. Selectee is subject to the NM pay while occupying this positions. NM Pay Range: $79,746-$142,569. Selectee will need to sign a Memo of Understanding that they occupy this pay range only as long as they remain in the Nurse Manager Role.
The Nurse Manager of the Home Based Primary Care (HBPC) Program manages all aspects of daily operations for the program, including but not limited to human, financial and other resources management; and, program development and strategic planning. The incumbent works collaboratively with the Medical Director and is responsible for ensuring that excellent Primary Care is provided to the Veteran population. Emphasis is placed on patient-centered, compassionate, quality care, delivered within a team approach to best serve the needs of each patient in a timely and efficient manner.
The Nurse Manager executes position responsibilities that demonstrate leadership, experience, and creative approaches to management of complex client care. The incumbent promotes patient and family health education with a focus on self-management, prevention, and wellness, based on the goals. As a change agent, promotes an environment that embraces new knowledge, innovations and improvements in our health care delivery systems. He/She implements new programs or enhancements consistent with local, VISN and VHA-national goals and priorities. Fundamental aspects of the role include: management and leadership in the care of patients; standardization of care; identification, delegation and oversight of care delivery and outcomes. The manager uses and refines data systems to identify trends and leads teams in the pursuit of ongoing opportunities for improvement.
