The normal tour of duty is Monday through Friday, 8:00 a.m. to 4:30 p.m. There may be some instances where the incumbent would be required to work after 4:30 p.m., or have the flexibility to work outside the normal tour of duty. Introduction: HUD-VASH is an intensive case management program for chronically homeless veterans aimed at providing psychosocial services to veterans while residing in stable housing through HUD's Section 8 voucher system.
The incumbent in this position functions as the Case Manager for Hampton VAMC's Housing and Urban Development/VA Supportive Housing (HUD-VASH). The HUD-VASH Program falls within the Homeless Programs of the Behavioral Health & Sciences Care Line. The incumbent is responsible for coordination of case management services for veterans receiving housing provided through HUD Housing Choice vouchers.
The initiative is known as the HUD-VASH program and was authorized pursuant to Division K, Title II, of The Consolidated Appropriations Act, 2008 (Pub. Law 110-161) ("2008 Appropriation Act") enacted on December 26, 2007 (see proviso (7) under the heading "Tenant-Based Rental Assistance"). The HUD-VASH program combines HUD- VASH rental assistance for homeless veterans with case management and clinical services provided by the Department of Veterans Affairs (VA at this medical center and in the community. Ongoing VA case management, healthcare and other supportive services will be made available to homeless veterans at as many VA Medical Center (VAMC) supportive services sites across the nation.
Duties and Responsibilities: Duties and responsibilities include but are not limited to the following:
The incumbent is responsible for providing case management for a caseload of veterans in the HUD-VASH Program. The incumbent is responsible for reviewing agreements between landlords, Public Housing Authorities and the VA in establishing appropriate referral for HUD-VASH placements.
The incumbent performs site visits of HUD-VASH apartments to ensure that the veterans reside in safe environments in compliance with local housing codes.
The incumbent verifies the veteran's status, eligibility for VA medical care, and eligibility of program participation.
The incumbent is responsible for ensuring that homeless veterans are considered as primary participants for program acceptance.
The incumbent is responsible for collecting and submitting HUD-VASH Program participant data as outlined by HOMES evaluations procedures.
The incumbent is responsible for providing HUD-VASH case management. Case management is defined as the provision of services by VA clinical staff to homeless veterans and includes all activities to provide appropriate treatment and maintain veterans in HUD approved stable housing.
The incumbent coordinates care including serving as an advocate on behalf of the veterans to ensure comprehensive service delivery, and linking and referring veterans to the VA Medical Center, VA Regional Offices and /or community agencies.
The incumbent complies with criminal conflict of interest laws and Executive Branch Standards of conduct so as to avoid conflicts of interest while serving as the HUD-VASH Case Manger.
The incumbent actively participates in the HUD-VASH Program conference calls and attends training offered by the HUD-VASH.
The incumbent is responsible for following local policy and SOP for the HUD-VASH Program. This includes following policies and documentation requirements as required for CARF and JACHO accreditation.
The incumbent actively participates in program planning and development, advising the supervisor of shifting trends with caseloads and identifying the need for making recommendations on program procedure or policy changes based upon work experiences.
The incumbent takes responsibility for improving professional skills, participating in planning and executing specific staff development projects, attending seminars and institutes, and participating in training programs of other disciplines as requested.
The incumbent performs accountability functions: 1) document all contacts with patient/families in the medical charts; 2) completes statistical and other reports in accordance with VA Medical Center and respective professional discipline service policies.
The incumbent works with VA Medical Center multi-disciplinary teams and community agencies to strive for developing more effective and efficient ways of delivering healthcare services to HUD-VASH Program veterans.
Performs other directly related duties as assigned
Physical Requirement. The incumbent's work is primarily sedentary. Infrequently, assignments may require prolonged standing. The incumbent may carry light items, such as books, files, or a laptop computer. Tasks may require moderate walking and manual dexterity. Position requires a current driver's license and an ability to operate an automobile. Must be eligible for vehicle insurance.
