Duties: The duties for the LPN at the Fredericksburg Community Based Outpatient Clinic (CBOC) will include, but are not limited to, the assistance of patients with rehabilitation and medical/surgical needs for acute and chronic patients. Incumbent performs, reports and records assigned nursing care in the CBOC. Assists with minor procedures, including obtaining and arranging necessary equipment, positioning patient, ensuring conform and privacy, monitors patient during the procedure. Adheres to standards of practice and safe technique in preparation and administration of medication. Promptly recognizes urgent or emergent patient care situations, seeks assistance and effectively intervenes as directed when appropriate. Perform daily therapies to include wound care, medication administration, teaching patient and nutritional assessments. Assures a safe patient environment. The LPN is directly accountable to the registered professional nurse who has been assigned as the Charge Nurse. The Nurse Manager has overall administrative responsibility for the supervision and guidance of the LPN.
Work schedule: Normal work schedule will be the day tour of duty, Monday-Friday. Alternate tours are required as dictated by patient care needs, and clinic functionality. PHYSICAL REQUIREMENTS: The physical requirements of this position include lifting, prolonged standing and sitting, frequent bending, walking (distance), climbing (steps, ladder, etc, reaching (overhead, extensive/repetitive), repetitive motion and stooping. The mental/sensory requirements include recall, reasoning, problem-solving, hearing, speaking clearly, writing legibly, keyboarding, reading and logical thinking. The environment's pace can vary from steady to a sometimes fast pace requiring handling of multiple priorities, frequent, sometimes intense customer interactions, and the ability to adapt to frequent changes in a sometimes noisy environment. BASIC REQUIREMENTS
Citizenship: Citizen of the United States.
Education: Graduate of a school of practical or vocational nursing approved by the appropriate State agency and/or accredited by the Accreditation Commission for Education in Nursing (ACEN) at the time the program was completed by the applicant. Exceptions include: Health care education in the military service or training in the military service which is accepted by the licensing body in the jurisdiction in which the individual is licensed as qualifying for full LPN/LVN licensure will be accepted as meeting the education requirements for VHA employment; Individuals granted a license by a jurisdiction that does not require graduation from an approved school.
Licensure: Full, active, current and unrestricted licensure as a licensed practical or vocational nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or District of Columbia.
English Language Proficiency: Licensed practical or vocational nurses appointed to direct patient care positions must be proficient in [both] spoken and written English as required by 38 U.S.C. 7402(d), and 7407(d).
Preferred Experience: A minimum of four (4) years of LPN experience in an acute care or clinic setting strongly preferred.

GRADE REQUIREMENTS: Grade Determination. In addition to the basic requirements stated above, the following qualification criteria must be met for the GS-6. The candidate's qualifications must clearly demonstrate the level of competence required for the grade.
GS-6:
(a) Completion of at least one (1) year of additional qualifying experience at the GS-5 level or equivalent, fully meeting all performance requirements for the GS-5 LPN/LVN.
(b) Technically proficient in initiating, performing and completing assignedduties in providing care to variable patient populations.
(c) Knowledge and ability to appropriately carryout assigned patient care based on the patients' conditions; to use judgment inselecting the appropriate order and sequence of procedures and treatments; andto accurately recognize, report and record relevant] patient information. Completed work [should need] only a generalreview by a registered nurse [(RN) or physician (MD/DO)] for [ ] appropriatenessand conformity with [established policies/procedures.
(d)] [ ] Ability to observe, identify and respond to the patient's needs for[care, including] medication, [ ] equipment-assisted care and [patient/familyeducation]. In [organizing and delivering]care [ ], the [LPN/LVN recognizes and] considers [ ] emotional [, cultural,spiritual, socio-economic, and age-related] factors.
(e) Prepares and administers prescribed medications (oral, topical,subcutaneous, intramuscular and/or intravenous) and performs treatmentsaccording to established policies/procedures. (f) Knowledge and ability to recognize urgent or emergent patient caresituations, seek assistance of the RN and/or MD/DO, and initiate appropriateemergency interventions as directed.
(g) Knowledge and understanding of human behavior, patient motivations andreactions to situations, and ability to appropriately utilize this knowledge inworking effectively with patients, family members, and other staff.
(h) Establishes constructive relationships with individual patients andtheir families to elicit feelings and attitudes, and to promote positiverelationships, communication and socialization skills. Fosters an environment of respect forindividual patient and family rights to privacy and dignity in all aspects ofcare delivery. Effectively incorporatesknowledge and understanding of established customer service standards in allinteractions with patients, family members, and/or other internal/externalcustomers.
(i) Knowledge and skill in performing support duties for complex diagnostictests and/or specialized practices or procedures, which include preparing thepatient, assisting in the diagnostic examination, preparing and handlingspecialized instruments or other specialized equipment, and monitoring thepatient's condition before, during, and following the procedure. Serves as a preceptor in orienting,educating, and training less experienced LPNs/LVNs or NAs/HTs related tosupport duties for these more complex, specialized tests/procedures.
(j) Actively seeks out educational opportunities to enhance nursingknowledge and skills, sharing new knowledge gained with other staff to improveand advance nursing practice.
To be creditable, practical nursing experience (as an LPN or LVN) must be documented on the application and verified in an employment reference or through other [independent] means.
Part-time experience is credited according to the relationship it bears to the full-time workweek (e.g., an LPN/LVN who worked 20 hours per week, i.e., half-time, would receive 1 full-time workweek of credit for each 2 weeks of such service).
BLS/ACLS REQUIREMENTS : Current certification in BLS is required for all staff providing direct patient care, and police.
