VA Puget Sound Health Care System (VAPSHCS) is a Complexity Level 1a Facility and provides extensive primary, secondary, tertiary, and quatemary care in outpatient, inpatient, residential, and community settings. VAPSHCS is a mutli-site medical system that includes a major medical facility in Seattle, Washington, outpatient and residential facilities at American Lake Campus in Tacoma, Washington, and six Community Based Outpatient Clinics. VAPSHCS is an academic facility with significant teaching and research programs.
The Caregiver Support Coordinator (CSC) at VA Puget Sound Health Care System, administratively responsible for the clinical programming and daily operations of the program. The incumbents primary responsibilities are to provide caregiver, Veteran and staff education on caregiver issues; community outreach; resource development; continuous quality improvement activities; evaluation/consultation and limited clinical evidence based services and interventions. In addition the incumbent must be a skilled communicator capable of developing effective partnerships with local, state and federal agencies as well as non-governmental organizations.
The Caregiver Support Coordinator is practicing at the advanced level and has specialized knowledge of practice that can be differentiated from the independent practice social worker by the ability to expand clinical knowledge in the profession, administer a facility program, provide consultation and guidance to colleagues, role model effective social work practice skills, teach or provide orientation to less experienced social workers, develop innovations in practice interventions, and provide clinical supervision for social work licensure or certification. The Caregiver Support Coordinator has at least two years of experience in an area of specialized social work practice. The incumbent must be a License Clinical Social Worker, licensed by a state to practice at the advanced practice level. The incumbent will function in support of caregivers, Veterans and family members throughout the medical center and in community settings, to included but not limited to:
Physical Requirements: The physical demands of this position involve primarily walking, talking, driving, use of the telephone, etc. Strenuous physical activity is not required although the incumbent is expected to withstand pressure and stress as part of their day-to-day activities. Work environment:
The incumbent observes normal safety precautions while working primarily in an office type environment. The environment also includes home visits, visits to community agencies, visits to care giving facilities in the community etc., depending on the incumbent's specific assignment. Most of the time the incumbent operates in an environment that is well lighted, clean, and does not pose a hazard to personal safety.
Here is ongoing expose to individuals having psychiatric and physical illness and disabilities. These include acme infections, violent or aggressive behavior, and chronic psychiatric and medical problems. Light lifting, under 15 pounds,
