In addition to our main facility in Tomah, we offer services in four community-based outpatient clinics. These clinics are located in La Crosse, Clark County, Wausau, and Wisconsin Rapids. VAMC Tomah is one of eight facilities that comprise Veterans Integrated Services Network 12 (VISN12) which is administratively headquartered at Westchester, IL, and includes facilities in Illinois, Wisconsin, and Michigan.

The Community Living Center (CLC) Registered Nurse (RN) is responsible for practicing within licensed scope of practice, regulatory and organizational standards, and departmental scope of service. The position requires the knowledge, skills, and abilities to provide competent resident centered care and to utilize resources efficiently. RN is self-directed in managing complex resident situations, utilizes the nursing process in assessing, planning implementing, documenting and evaluating residents individualized care. The RN collaborates with physicians and other health care providers and utilizes the VA Core Values of Integrity, Advocacy, Respect, and Excellence (I CARE) with all interactions creating a Veteran centric care environment. The RN supports, educates, directs and guides the performance of LPN and CNA staff on the neighborhood. The RN ensures that care is timely, appropriate, of high quality and cost effective. The RN acts on behalf of the veteran to assure that necessary services are received and that progress towards resident established goals is being made. The RN works effectively with residents, families and significant others, providing support, in coping with emotional, mental, medical, and physical decline/impairment. The RN works with staff so morale remains high and staff turnover remains at a reasonable level. Areas of care include hospice, long-stay maintenance, and short stay skilled care.
The major duties and responsibilities include, but are not limited to:
Physical Requirements: Walking, bending, stooping, and some lifting in the direct care of patients. There may also be extended periods of sitting.
