A vacancy exists for a Clinical Outcomes Coordinator in the Community Living Center within Nursing Service. The incumbent is the designated second line supervisory authority on the unit. In conjunction with the CLC Nurse Manager, he/she is responsible and accountable for the 24/7 clinical operations of the unit, ensuring that subordinate personnel provide timely nursing care which compiles with generally accepted standards of clinical practice.
He/she acts in the absence of the Administrative Nurse Manager on one or more units for operational purposes including granting of leave. He/she will have second rating responsibility for RN staff assigned. The incumbent is qualified by education and experience to manage clinical outcomes on one or more units as assigned, ensuring safe and therapeutic care for the residents assigned to the unit. This will include but is not limited to the following responsibilities: supervision of subordinates, approved of leave and schedules in VATAS, rounding on residents, interacting with other disciplines, reviewing documentation, evaluation of care delivery, continuous survey readiness and other duties as assigned.
The incumbent possesses intimate knowledge of Long Term Care federal regulations, SAIL data, CARF standards, VHA guidelines, medical review criteria, Minimum Data Set, clinical documentation requirements, current standards of care, and compliance guidelines. The incumbent is aligned within the Department of Geriatrics and Extended Care, Community Living Center under Nursing Service and is responsible for providing the clinical leadership using an Evidence Based Model and clinical expertise for the coordination of care provided to the Veterans within the Community Living Center.
The incumbent executes position responsibilities that demonstrate leadership, experience, and creative approaches to management of the long term resident. He/she demonstrates the ability to identify an issue, analyze it, and develop and implement interventions. He/she possesses skill sets for implementing health promotion and chronic disease management using clinical practice guidelines and performance measures. He/she utilizes all avenues of resources available to provide safe and efficient timely coordination of quality health care and interdisciplinary services. He/she performs other related duties as assigned.

Physical Requirements: This position requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity. This position requires potentially long periods of continued walking, standing, stooping, sitting, bending, pulling, and pushing. Wearing a lead apron while performing these duties will be required. Transferring patients and objects may be required. Mechanical inpatient lifts are provided. The incumbent may be exposed to infected patients and contaminated materials and may be required to don protective clothing in isolation situations, operative/invasive procedures, or home settings. The incumbent may occasionally be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders. The incumbent must be mature, flexible, sensible individual capable of working effectively in stressful situations, able to shift priorities based on patient needs. Must complete annual Employee Health requirements, such as annual TB screening or testing, as a condition of employment. The incumbent who provides care in home settings or other off-site locations may be required to drive and/or ride GSA vehicles.
During this current flu season, all health care providers selected from this job announcement will be required to receive the influenza vaccine or provide proof of vaccination by November 30 or within two weeks of beginning employment (if after November 30th).
