The Community Living Center Restorative Nurse Coordinator (RNC) is self-directed nurse leader and executes position responsibilities that evaluates resident's underlying capability with regard to function and physical activity, and assures that residents optimize and maintain their highest possible level of functioning and increase time spent in physical activity. The RNC uses clinical knowledge/judgment based on the nursing process to promote staff and family involvement in the assessment planning, implementation, documentation, and evaluation of Veteran care outcomes. The RNC is an integral part of the CLC interdisciplinary team and identifies the learning needs of direct care staff and provides formal and informal education and evaluation of their progress towards their goals. The RNC is a clinician who demonstrates the ability to work effectively with the geriatric Veteran population, families, or significant others, and with licensed professionals and frontline staff. The RNC serves as CLC a resource and an integral member of the interdisciplinary team who utilizes the VAPAHCS Strategic Plan Priorities: Safety, Quality, People, Innovation, Access and Stewardship to lead and or support all members of the IDT. Is actively involved in quality outcomes that are pertinent to the CLC veteran population and collects and/or reviews data from the following areas: RAI/MDS, Assaults, Catheter Associated Urinary Tract Infections (CAUTI), CareTracker ADL and Restorative Reports, Falls, Hospital Acquired Pressure Injuries, Pain Management, Restraints and TruthPoint Resident Surveys. The RNC utilizes VAPAHCS tools and opportunities for effective communication including the Daily Management System (DMS), the SERVE Model (Say Hello, Engage, Respond, Verify, and Exit) and the Service Improvement Model (SIMS) to achieve excellent customer service for veterans, staff and our external customers.
The major duties and responsibilities include, but are not limited to:
Physical Requirements: This position requires heavy lifting, 45 pounds heavy carrying, 45 pounds straight pulling (2 hours); reaching above shoulder; use of fingers; both hands required; walking & standing 8 hours - intermittently; repeated bending; both legs required; ability for rapid mental& muscular coordination simultaneously; near vision correctable at 13" to 16" to Jaeger 1 to 4; far vision correctable in one eye to 20/20 and to 20/40 in the other; depth perception; ability to distinguish shades of colors; hearing (aide permitted). Transferring patients and objects may be required. The incumbent may be exposed to infected patients and contaminated materials and may be required to don protective clothing in isolation situations or operative/invasive procedures. The incumbent may occasionally be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders. The incumbent must be a mature, flexible, sensible individual capable of working effectively in stressful situations, able to shift priorities based on patient needs.
