One full-time Social Worker, GS-0185-11 position is currently available in Mental Health & Behavioral Sciences Service. Services are provided in the Hampton VAMC Mental Health Intensive Case Management (MHICM) Clinic. The normal tour of duty is Monday through Friday, 8:00 a.m. to 4:30 p.m. There may be some instances where the incumbent would be required to work after 4:30 p.m., or have the flexibility to work outside the normal tour of duty which may include weekends and holidays. Introduction. The Clinical and program activities assumed under this positon are conducted as part of the Mental Health Intensive Case Management Program (MHICM). The goal of the MHICM is to develop, implement and assess the benefit of a behaviorally oriented program or psycho social rehabilitation, intensive case management and community based treatment for chronically mentally ill veterans who have been high uses of Inpatient Mental Health services. Work is done in the context of an interdisciplinary treatment team, and extensive liaison activities with other interdisciplinary teams are required to provide continuity of care for assigned veterans in outpatient settings. The incumbent must have a current, valid, unrestricted driver's license to operate a GSA vehicle for community home visits and outreach activities.

Duties and responsibilities include but are not limited to the following:
Clinical Functions: Incumbent is a professional social worker whose duties and responsibilities are centered on provision of psychosocial rehabilitation and intensive case management series to chronic, mentally ill Veterans enrolled in the Mental Health Intensive Case Management (MHICM). The incumbent must use a high level of skill in assessing and treating Veteran who are experiencing a variety of psychiatric, medical and psycho social problems. Direct contacts are provided to the Veterans and their families in the home frequency may increase during times of crisis. Social Work case management and practice which includes psycho social assessment, diagnosis and treatment is focused on helping the Veterans and their families maximize their potential to maintain independent living.
Must have a high level of skill and expertise to establish and maintain effective therapeutic relationships with Veterans in the MHICM Program and their families; able to independently work with Veterans and their families who are experiencing a wide range of complicated, medical, psychiatric, emotional, behavioral and psycho social problems; independently complete through psychosocial assessments to determine the psychosocial functioning and needs of Veterans and their families; be able to utilize this assessment in facilitating the Veterans maximum use of treatment for attainment of the highest level of independence that is possible and practicable; actively participate as a member of the interdisciplinary treatment team and actively participate through collaboration with Veterans and family as well as interdisciplinary treatment team members in the development and implementation of an individualized, patient centered treatment plan that focuses on strengthening a Veterans ability to address acute and chronic problems that have resulted in past history of readmission to the medical center or in the community; possess a working knowledge and experience in use of medical and mental health diagnoses, disabilities, and treatment procedures including acute, chronic and traumatic illnesses, common medications and their effects/ side effects, and medical terminology; facilitate continuity of care by collaborating and providing consultation with the referring interdisciplinary treatment team to promote appropriate treatment and rehabilitation in discharge planning; serve as a liaison between the Veterans and/ or their families and VA and community resources to ensure through delivery of services; possess knowledge and ability to independently implement treatment modalities, to provide social skills training, to provide patient education, and to provide counseling and/ or supportive groups for Veterans and families; provide consultation and education to Veterans and their families regarding community resources, VA benefits and specialty programs, and advance directives; be available to provide consultation to other treatment team and staff members regarding psychosocial needs of the Veterans and/or their families and the impact of the identified psychosocial problems on the Veterans health care planning, compliance with treatment and discharge planning.
Administrative Functions: Is responsible for supporting the mission, policies, and procedures of the Department of Veteran Affairs, The Veterans Health Administration (VHA) and appropriate Veterans Integrated Service Network (VISN) and the facility; completes the various documentary requirements of the program in accordance with policy guidance. Guidance can be provide locally or interpreted by VISN and/or National MHICM programming. Compliance to the standards is in keeping with any accreditation required by the Medical center, VISN or service line. This may include the Joint Commission (JC), Commission on Accreditation of Rehabilitation Facilities (CARF), Office of Inspector General (OIG), or any other accreditation body sanctioned as appropriate by the VHA; compete various NEPEC forms and MHICM documentation accurately and in a timely manner; is mindful of keeping his/ her supervisor apprised of problems and recommended solutions to problems encountered in the incumbent's area of responsibility; responsible for furthering one's own professional growth through continuing education and for ensuring continuing education appropriate to the area of assignment; able to provide assistance with the orientation and learning of less experience social workers and if interested to serve as a preceptor for social work students if the incumbent possess the appropriate credentials to be a preceptor; assist in the orientation of new staff to the MHICM team; able to maintain a level of productivity and quality consistent with the complexity of the assignment and consistent with Social Work Standards, Joint Commission, CARF standards and facility standards.

Citizenship. Be a citizen of the United States. (Non-citizens may be appointed when it is not possible to recruit qualified citizens in accordance with chapter 3, section A, paragraph 3g.
Education. Have a master's degree in social work (MSW) from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the school of social work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Applicants must submit a copy of their transcript(s) to verify education
Licensure. Persons hired or reassigned to social worker positions in the GS-185 series in VHA must be licensed by a state to independently practice social work at the master's degree level. Current state requirements may be found on the OHRM website.

English Language Proficiency. Social workers must be proficient in spoken and written English in accordance with VA Handbook 5005, Part II, chapter 3, section A, paragraph 3j, this part.
Experience, Education and Licensure. Promotion to the GS-11 full performance level requires completion of a minimum of 1 year of post-MSW degree experience in the field of health care social work (VA or non-VA experience) and licensure or certification in a state at the independent practice level. The VA may waive the requirement of licensure up to the GS-11 level OR In addition to meeting basic requirements, a doctoral degree in social work from a school of social work may be substituted for the required 1 year of professional social work experience in a clinical setting.
