Duties:
The Licensed Practical Nurse Provides a full range of Practical Nursing care to patients with a variety of physical and/or behavioral problems.
Assists with and transfers patients in, to, and from the exam table, wheelchair, chair, and/or stretcher. Escorts patients to appointments within the hospital and off-station as assigned. Utilizes safe patient handling (SPH) equipment as appropriate to prevent injury to peers, patients and self.
Prepares, administers and appropriately documents actions taken specific to commonly prescribed oral, topical, subcutaneous, intramuscular, intradermal, and/or intravenous medications as permitted by approved local facility policies and procedures.
Performs treatments for patients using medical and/or surgical aseptic techniques, according to defined local facility policies and procedures for wound care, suture/staple removal, setting up sterile fields, and assists physician or registered nurse with other procedures.
Completes clinical reminders as appropriate, measures, reports, and/or documents vital parameters of body functions and changes in physical, mental, or behavioral states as indicated. Notifies registered nurse and/or provider of parameters that are outside of the normal range.
Collects, tests, and/or processes specimens of body excretions, tissue, and/or secretions according to established procedures.
Observes and documents the physical, mental, and behavioral states of patients in accordance with defined procedure.
Assists with daily clinic flow and efficiency by checking in and rooming patients, obtaining and documenting vital signs and the chief complaint, facilitating patient care needs, assisting with in-clinic procedures, and chaperoning patient exams as needed.

1. At least one to two years clinical experience as an LPN required.
2. Mental Health or Outpatient nursing experience desired.

5. Ability to collect and record a variety of patient data in a computer based electronic medical record system.

Grade Determinations:
GS-3
(A) Must meet basic requirements listed above (Licensure, citizenship, education,etc.) LPNs at this grade level are expected to perform all duties within thescope of their license (e.g., medication administration); however, completedwork may require routine or detailed higher level review depending upon thecomplexity of the duties involved.
GS-4:
(A) Six months of qualifying experience as an LPN or LVN.
(B) Graduation from an approved school and one year of experience that involvednursing care work in a hospital, outpatient clinic, nursing home, or othersupervised medical, nursing, or patient care facility that provided a practicalknowledge of human body structure and sterile techniques and procedures.
GS-5:
(A) Completion of at least 1 year of qualifying experience at the GS-4 level or equivalent.
(B) Demonstrated knowledge and ability to provide a full range of practical nursingcare to patients with a variety of physical and/or behavioral problems; demonstrated ability to serve as a responsible member of the nursing team and interact in an appropriate manner with patients, family members, professional and other supportive personnel involved in the delivery of patient care, incorporating acceptable, established customer service standards into practice; knowledge and skill sufficient to prepare, administer, and appropriately document actions taken specific to commonly prescribed oral, topical, subcutaneous,intramuscular, and/or intravenous medications as permitted by approved local facility policies and procedures. Observation and documentation will include patient's response to medication administered and the reporting of any notedchange in patient's condition to RN or MD/DO; knowledge and ability torecognize the need for and to institute emergency measures when indicated, promptly seek the assistance of the RN or MD/DO, and assist in resuscitation proceduresin cardiac and/or pulmonary arrest; recognizes and appropriately responds to breakage/malfunction or loss of equipment, safety hazards, and supply deficiencies, promptly reporting to appropriate personnel for corrective action;completed work is under the general supervision of an RN or MD/DO.
GS-6
(A) Completion of at least one year of additional qualifying experience at the GS-5level or equivalent.
(B) Demonstrated knowledge and ability to appropriately carry out assigned patient care based on the patients' conditions; to use judgment in selecting the appropriate order and sequence of procedures and treatments; and to accurately recognize, report and record relevant patient information. Completed work [should need]only a general review by a registered nurse [(RN) or physician (MD/DO)] for appropriateness and conformity with established policies/procedures.
NOTE: The full performance level of this vacancy is GS-6.The actual grade at which an applicant may be selected for this vacancy is inthe range of GS-3 to GS-6.
