As a Physician Assistant or Advanced Nurse Practitioner within the Patient Aligned Care Team (PACT) Section of the Primary Care Service Line, the incumbent will provide comprehensive outpatient care to a cohort of patients shared with an overseeing Physician. The incumbent will be fully trained in the PACT model of care, either by previous training or during their initial tenure with the Black Hills Health Care System.
Duties will include, but are not limited to the following:

LOCATION: Primary & Specialty Care Service Line, VA Black Hills Health Care System. Vacancies exist at the Fort Meade, SD hospital and the Rapid City, SD Community Based Outpatient Clinic.

PHYSICAL REQUIREMENTS:
