Officially established in October 2006, the Orlando VA Medical Center serves more than 105,000 Veterans in Central Florida and includes a 120-bed Community Living Center, two Mental Health Residential Rehabilitation Treatment Programs/Domiciliaries (60-beds located at the Lake Nona Campus and 58-beds at the Lake Baldwin Campus), three large multi-specialty Satellite Outpatient Clinics in Orlando (Lake Baldwin campus), Viera and Daytona Beach, and four Community-Based Outpatient Clinics located in Clermont, Kissimmee, Orange City, and Tavares. The new medical center at Lake Nona is open for outpatient and inpatient services and will continue to expand inpatient beds, Emergency Services, and other specialty services throughout 2017. The Orlando VA Medical Center is seeking a Registered Nurse (RN)/Case Manager to facilitate and coordinate between members of the Medical Center and non-VA providers in the local community for outpatient services that cannot be provided by the VA. The incumbent will process consults - utilizing clinical judgment and VA guidelines to determine if services can be provided inter-facility or need to be referred for non-VA care. This Case Manager functions independently, demonstrating decision making capacity and is responsible for contacting non-VA providers for authorization of medical care at VA expense. He/she will collaborate with the veteran and interdisciplinary staff members to maximize healthcare responses, quality and cost effective outcomes. This RN provides follow-up and ensures continuity of care during the time the veteran is receiving non-VA services. A variety of administrative functions are necessary for smooth management, including but not limited to: correspondence/ telephone calls, faxing, tracking tools. The incumbent reports to the Nurse Manager, Care Management
Work Schedule: Normal work days/hours will be Monday-Friday, 8:00 AM to 4:30 PM, with work day/hours flexibility required as administrative and/or clinical demands require.
