The RN, Quality Management Coordinator serves in the Rehabilitation and Extended Care Line within the Extended Care Line, under the supervision of the Extended Care Nurse Executive. This individual will provide primary oversight and consultative services toward organizational continuous readiness for the following accrediting/monitoring, organizations/activities:
§ Joint Commission on Accreditation of Healthcare Organizations (JC)
§ Hospital Manual
§ Long Term Care Manual
§ Behavioral Health Manual
§ Home Care Manual
§ Office of Inspector General (OIG)
§ Long-Term Care Institute (LTCI)
§ System-wide Ongoing Assessment and Review Strategy (SOARS)
§ Medical Center Assessment and Audit Program (MAAP)
The incumbent will assist the facility Quality Manager and top management as required, in policy review, formulation and communication, implementation, and evaluation of the accreditation process.
The incumbent develops programs to insure that quality provisions are planned, developed and implemented to identify, prevent and correct unsatisfactory conditions and elements which influence the regulatory correctness and responsiveness of transactions and services. He/she tailors quality program management strategies and techniques to fit the operations, culture and interests of the installation's management and staff and develops policy guidance, motivational materials and information to facilitate participation in important program initiatives. Additional duties include but are not limited to:
· Demonstrates ability to apply complex problem solving skills at the program level, and use team approaches to identify, analyze and resolve problems.
· Displays leadership in identifying and evaluating issues in order to improve clinical programs and how long term care is delivered.
· Initiates/actively participates on interdisciplinary committees or groups that evaluate clinical practice or health
services.
· Demonstrates leadership and innovation in planning for, providing and improving the delivery of care and the
development of staff as it relates to long term care initiatives, such as SAIL measures.
· Initiates policies, standard operating procedures, guidelines, performance improvement activities, and performance measurements relative to practice area.
· Ability to work effectively with and as a part of the interdisciplinary team. Skilled at fostering confidence and
cooperation within and among staff, physicians, and administrative leaders.
· Ability to utilize sound judgment in making administrative decisions including the ability to accurately interpret VAMC policies and regulations.
· Ensures all staff completes MDS competency training upon hire and annually; and facilitates monthly unit in-service education programs.
· Ensures staff compliance with Service Line goals and Network performance standards for MDS and SAIL measures. .
· Assists interdisciplinary team in following guidelines for planned change and in using quality improvement data when proposing change or improvement in nursing care.
· Ability to successfully navigate through accrediting and VHA initiated surveys.
· Ability to interpret, implement and sustain long term care standards in relative practice areas.
· Provides leadership coverage as needed.
Work Schedule: Day Shift, 7:30am-4:00pm; flexible 8hr tours, requires after hour tours, including some weekends and holidays.
