The Telecare Registered Nurse (RN) provides compassionate and safe healthcare through telecommunication, working in collaboration with Providers and other Disciplines to maintain continuity of patient care, wellness and customer satisfaction. The Telecare RN is a self-directed professional who uses the nursing process and evidence-based practice to meet the needs, questions or concerns of the Veteran, their representative, Visitors and VA staff. The selected RN may respond to a variety of calls ranging from calls from other healthcare facilities, request for appointments or medication refills, request for information or requests from ill or injured Veterans seeking medical assistance. The Telecare RN will respond to each call appropriately, assisting with the navigation of the health care system and intervenes or refers as appropriate to facilitate the achievement of patient outcomes. Telecare calls may range in length from 3 to 30 minutes for determination of the callers need(s), evaluation and appropriate response and documentation of encounter.
The Telecare RN is also responsible for providing post-discharge calls for recently discharged inpatients and follow-up calls to patients discharged without being assigned to a Primary Care Provider. Review of discharge instructions, as well as Social Worker summaries, are conducted for discharged patients.
Other duties include, but are not limited to the following:
1. Participation in Quarterly after-hours Call Center drills.
2. Providing coverage for calls received in Primary Care clinics as needed.
3. Member of Telecare Relocation Team assisting with calls during system malfunctions or natural disasters.
Work Schedule:3:30pm - 12:00am OR 11:30pm - 8:00am including weekends and holidays.
Physical Requirements:
The work is sedentary. Typically, the employee may sit comfortably to perform portions of the work. However, there is periodic walking and standing.
Grade Determinations:
NOTE: Applicant's education and length of nursing practice (experience) will be considered by a Nurse Professional Standards Board in determining the grade and step of the applicant selected. Experience as a RN will be evaluated to determine if the experience is of an acceptable level of quality with regard to the following four dimensions of nursing: Practice, Professional Development, Collaboration and Scientific Inquiry. Each dimension of practice has criteria that demonstrate essential core competencies for VHA Nursing Personnel.
The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:
Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required.
Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience.
Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience.
Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 years of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 years of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required.
Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 years of nursing practice/experience; OR a Doctoral degree in nursing or related field and approximately 2-3 years of nursing practice/experience.

Education/Experience Requirements:
Minimum of an Associate Degree in Nursing (ADN) or a Diploma in Nursing.
Preferred Experience/Education:
Bachelor's degree in Nursing (BSN) or a Bachelor's degree in a related field with an Associate's Degree or Diploma in Nursing.
Two (2) years of recent professional (RN) nursing experience.
Board certification from a VA approved certifying body.
