Specifically directed toward homeless veterans and their families or veterans at risk for homelessness, this counselor is responsible for program development, oversight, monitoring, administration and effectiveness, serving as the primary contact with local Public Housing Agencies (PHAs) and providing case-management based services (directly or through referral) necessary to ensure that veteran care is coordinated with the VA Medical Center, community health or Mental Health providers, and agencies that serve homeless populations. Duties include, but are not limited to:
o Assists in planning, establishing, implementing policies and procedures. The incumbent will assist the HCHV/Homeless Coordinator with overseeing the data collection and administrative record keeping, and monitors patient or program clinical indicators, thresholds, and performance measurements.
o Implements and maintains referral and screening procedures for potential HUDNASH veteran participants that meet national policy as well as the needs of the medical center mental health services and residential programs.
o Interviews veterans and their family members/significant others to establish facts about the veteran's situation, presenting problems and their causes, and the impact of such problems on the veteran's functioning and health as part of a comprehensive psychosocial assessment.
o Provides clinical services to veterans and family members/significant others in support of the veteran's treatment.
o Establishes a continuing relationship with the veteran, evaluating progress toward goals and adjusting the treatment plan as appropriate.
o Provides crisis intervention when indicated including evaluation, assessment, triage, initiation of/support in Emergency Detention procedures, and disposition of patients experiencing behavioral health emergencies. As a licensed behavioral health professional, is required to provide "on-call" consultative services during evenings, nights and weekends, on a rotating basis with other qualified MHSL staff. While "on call" the incumbent is available to respond, by phone and in person as indicated by the situation, to the medical center to provide "on call" consultative behavioral health services timely. These services include evaluation, diagnosis, assessment, triage, and supporting the emergency department, medical units and RRTP in the disposition (especially recommended level of care) for patients presenting with behavioral health emergencies during off hours.

Physical Requirements:
Pre-employment physical is required. The work is mostly sedentary and is performed in a clinic responsible for treating patients with a wide variety of medical problems. Normal safety precautions and practices are required for working a health care environment. This position requires emotional ability to deal effectively with multi-tasking and high volume of patient flow. Other physical demands include reaching, bending, stretching, walking, and carrying light loads of supplies.
Basic Requirements:
-U.S. citizenship. Non-citizens may be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
-Education. Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the school of social work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work.
-Licensure. Must be licensed or certified by a state to independently practice social work at the master's degree level.
-Grade Determinations for GS-11 Social Worker- requires completion of a minimum of 1 year of post-MSW degree experience in the field of health care social work (VA or non-VA experience) and licensure or certification in a state at the independent practice level.
OR
In addition to meeting basic requirements, a doctoral degree in social work from a school of social work may be substituted for the required 1 year of professional social work experience in a clinical setting. In addition, the candidate must demonstrate the KSAs below.
Required Knowledge, Skills, and Abilities (KSAs)
1. Knowledge of community resources, how to make appropriate referrals to community and other governmental agencies for services, and ability to coordinate services.
2. Ability to independently assess the psychosocial functioning and needs of patients and their family members and to formulate and implement a treatment plan, identifying the patient's problems, strengths, weaknesses, coping skills and assistance needed, in collaboration with the patient, family and interdisciplinary treatment team.
3. Ability to independently conduct psychosocial assessments and provide psychosocial treatment to a wide variety of individuals from various socio-economic, cultural, ethnic, educational and other diversified backgrounds. This requires knowledge of human development and behavior (physical and psychological) and the differential influences of the environment, society and culture.
4. Knowledge and experience in the use of medical and mental health diagnoses, disabilities and treatment procedures. This includes acute, chronic and traumatic illnesses/injuries, common medications and their effects/side effects, and medical terminology.
5. Knowledge of psychosocial treatment and ability to independently implement treatment modalities in working with individuals, families and groups who are experiencing a variety of psychiatric, medical and social problems to achieve treatment goals. This requires independent judgment and skill in utilizing supportive, problem solving or crisis intervention techniques.
6. Ability to independently provide counseling and/or psychotherapy services to individuals, groups and families. Social workers must practice within the bounds of their license or certification. For example, some states may require social workers providing psychotherapy to have a clinical level of licensure.
7. Ability to provide consultation services to other staff about the psychosocial needs of patients and the impact of psychosocial problems on health care and compliance with treatment. Ability to provide orientation and coaching to new social workers and social work graduate students. Ability to serve as a field instructor for social work graduate students who are completing VHA field placements.
8. Ability to independently evaluate his/her own practice through participation in professional peer
Preferred Experience: Knowledge and experience working with individuals experiencing homelessness or at risk for homelessness. Experience working in conjunction with community partners and systems in accessing local and state resources. Experience or ability to work from scheduled activity and to prioritize tasks when needed, which may entail deviation from a planned day.
Experience refers to paid and unpaid experience, including volunteer work done through National Service programs (e.g., Peace Corps, AmeriCorps) and other organizations (e.g., professional; philanthropic; religions; spiritual; community; student; social). Volunteer work helps build critical competencies, knowledge, and skills and can provide valuable training and experience that translates directly to paid employment. You will receive credit for all qualifying experience, including volunteer experience.
