The Clinical Pharmacy Specialist is a licensed professional with advanced knowledge and proficiency in pharmacy practice including pharmaceutics, pharmacokinetics, pharmacodynamics, and pharmacotherapeutics.
Practices as a mid-level provider within a scope of practice in providing care for patients requiring pain management services.
Utilizes telehealth modalities as necessary to help maximize access to care.
Designs, implements, assesses, monitors, and documents therapeutic plans utilizing the most cost effective, least toxic, and most effective therapy.
Completes an appropriate review of all medication orders to include selection of drug therapy based upon the pharmaceutical principles of pharmacokinetics and pharmacodynamics; monitors for efficacy, side effects and clinical outcome; and advises prescribers as appropriate.
Reviews all medication orders for appropriateness, drug selection, dosage, route of administration and the amount, and adherence to the VA formulary process.
Provides patient-specific therapeutic drug monitoring and communicates relevant findings and/or recommendations to other health care providers in charge of the patient both orally and in writing.
Performs continuous evaluation of prescribed medications to assure optimal drug therapy.
Completes health and medication histories including patient interviews.
Recommends formulary alternatives and completes non-formulary consults as assigned.
Documents recommendations and interventions.
Represents pharmacy and contributes clinical and administrative expertise as a member of committees within the assigned area.
Assures security over pharmacy areas and provides an immediate report to management in event of inconsistencies.
Work Schedule: Eight (8) hour tour between the hours of 7:00am - 5:30pm and occasional weekends and holidays.
