The Nurse MDS/Restorative Nurse Coordinator is a professional nurse with extensive clinical knowledge to effectively implement health promotion, prevention, management of acute and chronic illness, and care of the dying. The MDS/Restorative Nurse Coordinator is obligated to provide ongoing, comprehensive evaluation and improvement of clinical practice and health care delivery systems to meet the needs of clients in the Geriatric and Extended Care (GEC) areas including individuals, significant others, and/or communities. Practice is based on current and anticipated trends in restorative and rehabilitative nursing practice and health care. There is a commitment to lifelong professional growth and learning. The MDS/Restorative Nurse Coordinator demonstrates nursing leadership in delivering and improving holistic care through collaborative strategies with others. The MDS/Restorative Nurse reports directly to the Chief Nurse Geriatrics and Extended Care.
The MDS/Restorative Nurse works closely with the Nursing staff and residents of The Community Living Center (CLC) and possesses the knowledge and skills necessary to effectively apply all aspects of the nursing process within a collaborative, interdisciplinary practice setting. This broad based clinical knowledge enables the RN to effectively implement health promotion and prevention interventions; manage acute and chronic illness states, assist veterans in the attainment of optimal levels of functioning through rehabilitation, and provide supportive measures for the dying patient and their significant others. The duties for this role include, but are not limited to:
Assist with analyzing and utilizing aggregate and trended data to identify significant patient care problems to make improvement. ·
Prioritizes resident needs and makes ethically sound decisions to ensure safe resident care.
Coordination of the Resident Assessment Instrument/Minimum Data Set (RAl/MDS) process, ensuring that all disciplines participate in timely completion of the RAl/MDS assessment.
Communicate effectively with all CLC and GEC interdisciplinary services and team members.
Routinely conduct EMR audits, identifying deficiencies and discrepancies for the purpose of quality improvement and accuracy in RAl/MDS.
Produce and RAl/MDS schedule or calendar indicating which assessments are due for review/update. ·
Serve as a resource person in accurately interpreting and communicating RAl/MDS policies and procedures.
Daily tracks resident's movements and documents performance levels.
Provide educational activities to improve clinical knowledge and performance of staff and self to ensure rehabilitative and restorative elements are integrated into basic practice
Applies knowledge of diverse and restorative nursing procedures and uses safe, cost-effective measures to provide care.
Works effectively with residents, family and staff by developing innovative methods to resolve rehabilitative/restorative care conflicts, including coping with physical and mental limitations and other stressful situations. ·
Ensures that all disciplines participate in completing the RAl/MDS assessment.
Performs and educates staff in restorative and maintenance programs as directed by PT and records effectiveness of programs
Conducts initial assessments and develops and implements the restorative program and plans of care
Conducts and reports quarterly reviews for the restorative program
Acts as a resource to others.
Work Schedule: Monday-Friday, 7:30am-4:00pm. Overtime including evenings, weekends and Federal holidays may be required

Physical Requirements:
The population of male and female veterans served ranges from the adult age 18, to the elderly adult, age 65 and over. The Ralph H. Johnson VA Medical Center is a major interdisciplinary teaching facility serving medical, surgical, and neurological inpatients and outpatients. There is a large outpatient psychiatric population, with admissions for medical co-morbidities. This position requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity. This position requires potentially long periods of continued walking, standing, stooping, sitting, bending, pulling, and pushing. Transferring patients and objects may be required. The incumbent may be exposed to infected patients and contaminated materials and may be required to don protective clothing in isolation situations or operative/invasive procedures. The incumbent will be exposed to patients who are combative secondary to delirium, dementia or psychiatric disorders. The incumbent must be a mature, flexible, sensible individual capable of working effectively in stressful situations, able to shift priorities based on patient needs.
