The Nurse Practitioner (NP) - Clinical Navigator position is in the Hospitalist Section under the Medicine Service, at the VA Central CA Healthcare System (VACCHCS), Fresno, CA. The incumbent is able to analyze patient complaints and signs/symptoms of illness or injury, and implement preventive patient care measures, promote positive health care, and provide educational support for patients with complex problems. The NP will directly support all patients on the acute care wards (Medical/Surgical & SCU) and facilitate patient flow. In collaboration with other members of the Medical/Surgical Treatment Team, the Navigator is responsible for organizing, coordinating, and providing care coordination and care management services to all inpatients (Medical/Surgical & SCU). Major duties include, but are not limited to:
- Respond to nursing and other staff call on urgent/non-urgent issues on all hospitalized patients in an appropriate and professional manner using excellent communication skills.
- Participate on inpatient rounds, interfacing with multiple other services and coordinating patient care.
- Ordering appropriate laboratory tests, EKG, X-Ray-and other tests.
- Initiate, renew, or refill medications as appropriate.
- Recording findings and setting follow-up appointments/evaluations.
- Counseling patients and family on health problems and treatment regimen.
- Demonstrate expertise in making medical judgments and be an active member of the multi-disciplinary healthcare team promoting whole health concepts.
- Conduct comprehensive assessment of patients' physical, mental, and psychosocial needs upon admission and coordinate with team regarding coordination of care and discharge planning.
- Develop and implement an individualized discharge care plan on all new admissions and collaborate with all services to ensure plan of care is successful.
- Develop care plans to prevent disease exacerbation, improve outcomes, increase patient engagement in self-care, and minimize hospital and ED utilization.
- Utilize behavioral strategies to help patients adopt healthy behaviors and improve self-care in chronic disease management.
- Order DME (Durable Medical Equipment) needs through Prosthetic Service as early as possible prior to discharge as recommended by Physical Therapist and/or MD.
