The Palliative Care Coordinator (PCC) is primarily responsible for implementation of the Palliative Care and Hospice Service; ensuring high quality practices in the treatment of symptoms at the end of life through the coordination and integration of various nursing, medical and complementary interventions. This responsibility includes coordination of the efforts of the Palliative Care Consult Team (PCCT); involved in caring for patients at the end of life on the critical care, acute inpatient units, Community Living Center and outpatient program and serves as a consultant and resource person; development and implementation of educational programs; and development and implementation of specific measures to monitor the effectiveness of the program. The Coordinator also oversees the transition of palliative care/ hospice patients between inpatient to outpatient settings and is the liaison between the VA and community hospice agencies. The position is located within the Geriatrics and Extended Care (GEC) Service Line, Community Living Center.
ADDITIONAL QUALIFICATIONS:
· Interprets data reflective of the patient's (and/or family's) physical, psychosocial, emotional, spiritual, cultural, and educational needs.
· Demonstrates skill in verbal and written communication.
· Demonstrates ability to work effectively with all members of the health care team.
WORK SCHEDULE: Full Time Tour of Duty Monday through Friday, 80 hours per pay period.

In cases of graduates of foreign school of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing.
· The completion of coursework equivalent to a nursing degree in MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6)
· Current, full, active and unrestricted registration as a graduate professional nurse in a state, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia.
Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:
· Nurse I Level I An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required.
· Nurse I Level II An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR and ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience.
· Nurse I Level III An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with BSN and no additional nursing practice/experience.
· Nurse II A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 years of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required.
· Nurse III Master's degree in nursing or related field with BSN and approximately 2-3 years of nursing practice/experience; OR a Doctoral degree in nursing or related field and approximately 2-3 years of nursing practice/experience.

Preferred Experience: Five years of practice/experience providing palliative care, chronic disease symptom management, pain management, and coordination of care for complex patients. Program level management including data analysis, quality monitoring, dissemination of best practices, and interdisciplinary collaboration.
Preferred Education: Master of Science in Nursing (MSN)
