Position Summary:
The Traveling Veteran Coordinator (TVC) is a facility designated RN assigned to coordinate care for Veterans on extended travel with their anticipated or unexpected medical needs to preventing any disruption in their care. This is a shared responsibility between the preferred and alternate facilities to ensure continuity of care. The TVC serves as the primary point of contact for these Veterans and their families or caregivers and communicates with the health care team while leading complex care coordination and case management efforts. The TVC also collaborates with the PCMM Coordinator to ensure proper assignment of patients when transferring between facilities as well as for Multi-PACT assignments. Required to work in multiple corporate databases and computer programs.
The TVC is responsible for:
Coordinating care between the alternate and preferred facilities for traveling Veterans.
Documentation (even if historical) so that progress can be reviewed from either facility via EMR or remote data.
Developing and cultivating a network of support staff at the facility that can assist in care coordination, particularly for specialty services.
Collaboration with the PCMM coordinator to facilitate Multi-PACT assignments or transfer of care between medical centers.
Work Schedule: Monday thru Friday, 8:00am - 4:30pm. Overtime including weekends, evenings and Federal Holidays may be required.

Physical Requirements:
This position requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity. This position requires intensive hand dexterity due to potentially long periods of computer and telephone usage. The incumbent must be a mature, flexible, sensible individual capable of working effectively in stressful situations, able to shift priorities rapidly based on patient needs

· BSN Preferred
· Two years of Emergency Department, Urgent Care or Primary Care experience
· Two years of Travelling Veteran Coordinator experience preferred.
· Highly skilled professional who collaborates with the Primary Care team to coordinate needed services for patients, with a emphasis on preventive health care.
