This employee will function as a core member of the Patient Aligned Care Team (PACT) model which is a patient driven, interdisciplinary team based approach that delivers efficient comprehensive and continuous care through active communication and coordination of health care services. Duties as assigned in order to support patient aligned care team principles identified as patient driven, team based, efficient, comprehensive, continuous, well communicate and coordinated care. Summary of PACT duties may include:
· Customer service- initial point of contact, patient advocate, address customer service concerns and coordinates solutions, hand off communication
· Assists providers- prepare paperwork requested by the Veteran and/or PCP, specialty consult completion tracking, coordinate information exchange the co-managed patient, manage telephone demand, manage appointment scheduling including recall pre-visit patient reminder calls as necessary
· Face to face visits- appointment check in, assist with My HealtheVet registration, perform in person authentication, assists with updating and verifying demographics and insurance information
· Teamwork- daily huddles and team meetings
· Clinic support- identifies and prepares required forms, documents/records prior to clinic session, faxing/copying/mail management, manage clinic grids, manage office supplies and setup
· Working collaboratively in an interdisciplinary coordinated care delivery model responsible for proper and timely treatment of patients
· Scheduling medical appointments using an electronic scheduling system and coordinating diagnostic testing
· Performing daily reviews or active/pending consults, electronic wait list, recall list and Audiocare
· Adjusting appointment times, location, or dates as well as shift patients to other health care providers as conflicts with staffing and/or coverage occurs
Work Schedule: Tours are based on operational need between the hours of 7:00am and 6:15pm.

Experience: Six months experience of clerical, office or other work that indicates the ability to acquire the particular knowledge and skills needed to perform the duties of the position

Experience/education combination: Equivalent combination of experience and education are qualifying for entry level for which both education and experience are acceptable.
Grade Determinations:
GS-6 Advanced MSA
Experience. One year of experience equivalent to the next lower grade level. Examples of experience equivalent to the next lower grade level work would be the ability to operate computerized programs and databases in order to enter, modify and retrieve sensitive information/data into or from electronic medical records, scheduling systems and/or reports; knowledge of basic medical terminology; ability to make appointments in a clinical setting; ability to work independently in the accomplishment of a wide variety of duties, including setting priorities and coordinating work; ability to communicate effectively and professionally with employees at varying grade levels; and ability to identify customer's concerns, perform the tasks required to resolve the issue accurately and timely, and follow-up as necessary to ensure a satisfactory resolution.
In addition to the equivalent experience at the next lower grade level qualified applicants would also need to demonstrated the following Knowledge, Skills, and Abilities below:
1. Ability to collaborate, communicate, set priorities, and organize the work
in order to meet

writing, in order to meet program objectives. This may include preparing
reports in various formats and presenting data to various organizational
levels.
3. Advanced knowledge of the technical health care process as it relates to
access to care.
4. Advanced knowledge of managing a clinic. This includes independently
utilizing reference
sources, decision making, and empowering the team to collaborate and resolve
problems within a complex systems environment.
5. Advanced knowledge of policies and procedures associated with operational
activities that affect the patient flow, patient care, and the revenue process.
6. Advanced knowledge of medical terminology due to the technical nature of
language utilized by clinicians.
