The RN Care Manager uses the nursing process and evidence-based practice to collaborate and communicate with the Core Teamlet (Veteran, Primary Care Provider (PCP), RN Care Manager, LPN, and Administrative Clerk), Tier II disciplines (Pharmacist, Social Worker, Dietician, Behavioral Health), and Expanded Team members (family/care giver, internal and community-based services involved in providing care to the Veteran) in developing a patient-centered health care plan for the Veteran's life. As a member of the Teamlet, the RN Care Manager is responsible for collaborating with services internal and external to the VA to facilitate care transition in order to effectively meet with Veteran's needs. The nurse is accountable for clinic responsibilities which included the proper intake and exit of the Veteran at the clinic visit, meeting educational and psychosocial assessment and needs, procedural assisting and appropriate assessing of telephone calls, walk-ins and unstable Veterans. The RN may be assigned responsibilities as a preceptor, team leader or Lead RN. The incumbent will work in partnership with their assigned PCP in managing their assigned panel of Veterans for effective disease prevention/health promotion and chronic care management. The RN Care Manager provides Veteran and family health education with a focus on self-management, prevention, and wellness, based on the Veteran's goals. The RN Care Manager serves as an advocate for Veterans, team player to colleagues as he/she continues to develop his/her own professional growth, development and practice.

Grade Determinations: Experience as an RN will be evaluated to determine if the experience is of an acceptable level of quality with regard to the following four dimensions of nursing: Practice, Professional Development, Collaboration, and Scientific Inquiry. Each dimension of practice has criteria that demonstrate essential core competencies for VHA Nursing Personnel. The criteria are as follows:
Preferred Experience: 5 Years of RN experience.
Experience refers to paid and unpaid experience, including volunteer work done through National Service programs (e.g., Peace Corps, AmeriCorps) and other organizations (e.g., professional; philanthropic; religions; spiritual; community; student; social). Volunteer work helps build critical competencies, knowledge, and skills and can provide valuable training and experience that translates directly to paid employment. You will receive credit for all qualifying experience, including volunteer experience.
Note: Only education or degrees recognized by the U.S. Department of Education from accredited colleges, universities, schools, or institutions may be used to qualify for Federal employment.
