The Community Care Nurse is the liaison between Veterans and agencies in the community. He/She helps to coordinate care, discusses clinical situations and billing questions in the community. The nurse spends much time explaining the process, doing clinical follow up as well as administrative follow up to ensure records are received within a timely manner so care can be transferred back to the primary care provider. The Community Care Nurse must have excellent clinical skills when discussing care with the Veteran to be able to alert them to changing to emergent conditions. The Community Care Nurse must demonstrate flexibility and creativity as the processes within the program are an ever-changing entity.
