The Licensed Practical Nurse in the Primary Care Clinic is an integral part of the Patient Aligned Care Team (PACT). The PACT LPN works under the direction of an RN and reports to the primary care manager. The LPN is responsible for observing, recording and reporting changes in the patient's condition, as well as assisting in the development and revision of the patient-driven holistic care plan for life. The LPN is a member of the larger Team. The LPN requires a general review of their work by an RN for appropriateness and conformity with license, policies and procedures. The LPN demonstrates active collaboration and communication with Teamlet (Patient, Primary Care Provider, RN Care Manager and Clinical Support/ Patient Service Administrative Assistant (PSA) and Larger Team (family/caregiver, internal and community based services and disciplines involved in providing care to the patient).This position makes observations and thoroughly documents care and the patient's responses. The incumbent will seek assistance and guidance by informing RN or medical provider of changes in the patient's condition requiring higher-level intervention. The LPN is responsible for observing, recording, and reporting changes in the patient's condition, as well as assisting in the development and revision of the patient-driven holistic care plan for life.
The LPN is required to possess the knowledge and skills to perform, report, and record assigned nursing care in the Primary Care Clinics, including, but not limited to:
Physical Requirements: The work requires a high level of active mobility with standing, walking, frequent bending, reaching and lifting. Heavy instrument trays, pieces of equipment and supply packages often weighing as much as 25 pounds must be lifted and carried. The employee regularly pushes and/or pulls loaded carts weighing several hundred pounds. Transport of heavily loaded carts must be negotiated through narrow passages, around corners, up and down inclines or uneven floors, over thresholds and in and out of elevators.
