The Clinical Pharmacy Specialist (CPS) position for pain management at the Tomah Veterans Affairs Medical Center (VAMC) functions in the clinical sections of the Pharmacy service line under the Associate Director's Office. This position is responsible for providing pharmaceutical care to Veterans with pain management issues. The incumbent strives to consistently provide pharmaceutical care to Veterans, optimizes Veteran care outcomes, functions at the highest level of clinical practice, and works independently under their scope of practice as defined by the Tomah VAMC to directly care for Veterans. The incumbent must be sensitive and concerned about what our customers believe is good service and attempts to meet or exceed their expectations. This position is not suitable for telework. The CPS reviews and evaluates requests for non-formulary (NF) and restricted drugs for appropriateness and compliance with established criteria where applicable. The CPS functions independently under a scope of practice as aforementioned for selected disease states related to pain management to provide pharmaceutical care to Veterans. This includes, but is not limited to, the following: initiating, modifying or discontinuing medication therapy; ordering labs and other tests to help determine efficacy or toxicity of medication(s); designing, implementing, assessing, monitoring, and documenting therapeutic plans. The CPS manages therapeutic recommendations independently under their scope of practice for pain management and related disease states according to best evidence based practice or VHA published guidelines. The CPS collaboratively monitors Veterans with the prescriber including adjusting medication(s) and ordering laboratory tests to ensure effective and safe therapy. The CPS will serve as a member of various committees, including Pain Committee and Opioid Safety Initiative. The CPS supervises and provides training for pharmacy students. The CPS will work collaboratively with an interdisciplinary pain team composed of a variety of clinicians. Veteran centered care delivered in a biopsychosocial, or whole health, perspective that empowers Veterans to make informed decisions is a key focus of our pain management delivery. This will include attending clinical pain team meetings, teaching group classes to Veterans, and contributing to ongoing staff education specific to the CPS area of expertise. The CPS performs other related duties as required.

Basic Requirements:
CITIZENSHIP: You must be a United States Citizen.
ENGLISH PROFICENCY: You must be proficient in spoken and written English.
EDUCATION: You must be a Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree.
OR
You must be a Graduate of a foreign pharmacy degree programs that meet the educational requirement and am able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL iBT).
LICENSURE: You must possess a current, full, active, and unrestricted license to practice pharmacy in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or in the District of Columbia.
SPECIALIZED EXPERIENCE: You must have the equivalent of 1 year of current creditable experience at the next lower level (GS-12) or equivalent, which is directly related to the position to be filled. You must possess one year of experience comparable to the GS-12 Level AND meet the KSAs required at the GS-12 and GS-13 Level as described in the Qualifications/Evaluations section of this announcement.

You must have completed 1 year Post Pharm D. ASHP accredited Residency.

Competency 2 - Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy.
Competency 3 - Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters.
