The RN Home Health Referral and Community Care Coordinator functions under the Home Care Supervisor appointed by the Chief of Staff and is responsible for adhering to established policies and procedures, demonstrating knowledge and leadership when providing nursing care, participating in the development and implementation of programs involving community nursing care. He/she is also responsible for providing clinical assessment, patient advocacy, and uniquely provide the pre-discharge education for patients who will be discharged to the home setting with a PICC line and home infusion of antibiotics, TPN, chemotherapy, hydration and/or line maintenance. Must maintain Infusion Nurse certification as well as a current PICC line placement and maintenance competency. Participates with the identified interdisciplinary team composed of the medical center, nursing, social work,, rehabilitation and dietetic services through inpatient team conferences, telephone access, CPRS referrals and written requests. Responsible for documenting interdisciplinary contacts and will attend appropriate discharge planning meetings. Acts as a liaison between the facility, Home Infusion Pharmacy, and the Home Healthcare Agency to provide Veteran's health. Responsibilities include data base reporting, record keeping, and ongoing fee service authorization for home healthcare as required by VA policies. Must be aware of the roles of the case manager in the effective use of home health care resources including Hospice, and Community Nursing Home placement to support the fragile VA Medical Center patient and family.
WORK SCHEDULE: Incumbent will normally work a Monday- Friday 8:00 a.m. to 4:30 p.m. schedule; however, schedule may be adjusted to meet the needs of the organization.

- Must be a graduate of a school of professional nursing approved by the appropriate State accrediting agency at the time program was completed by the applicant.
- Must have experience with Diabetes, COPD, CHF, Depression, and Hyper tension.
- Must be willing to complete training to provide back up for the RN's who pride Case Management using Home Telehealth technology using the Disease Management Protocols.
- Must be willing to obtain additional competency in Telehealth supported by on-line,conference calls based and face-to-face training.
- Prefer 2-3 years with Case Management and Discharge Planning.
Special consideration will be given to applicants with experience in home telehealth care or public health nursing, nurses with case management/fee service training, and nurses with leadership training or demonstrated competence in these areas who are self-motivated, flexible, and possess excellent patient documentation skills.
REGISTRATION:
Full, active, current and unrestricted registration as a graduate professional nurse in a State, Territory, or Commonwealth of the U.S. or in the District of Columbia.
EDUCATION:
BSN; or
Associate Degree or Diploma in Nursing and Bachelors' degree in a related field; or
Master's Degree in nursing or in a related field with BSN (if MSN obtained in a Bridge Program, no BSN required); or
Doctoral Degree in nursing or in a related field.
SPECIALIZED EXPERIENCE: Includes but is not limited to experience with Joint Commission, the Office of Inspecting General and/or the Telehealth Conditions of Participation. Program building, integration, and training of RN and technical staff in either Home Telehealth technology using the Disease Management Protocols. Experience with Diabetes, COPD, CHF, Depression, and Hyper tension.
