The Home Based Primary Care Nurse Practitioner (HBPC NP) is located within the Ambulatory Care Service Line and functions in an advanced practice role in directing interdisciplinary programs. The HBPC NP leads and supervises interdisciplinary teams in developing methods to community services, often the frail, at-risk, elderly veteran population. The incumbent assures that the care in the HBPC program meet the guidelines established by the VA Headquarters, VAMC CPM's, and JC standards. The HBPC NP evaluates the outcomes related to effectiveness and efficiency of care. The HBPC NP participates in planning the delivery of patient care with all administrative and clinical leaders.
The HBPC NP functions as an independent member of the Medical Staff with delineated clinical; privileges granted by the medical Staff. The NP is a member of the interdisciplinary team providing primary care for patients and/or employees.

BASIC REQUIREMENTS:
Certification Requirements: A nurse practitioner must be licensed or otherwise recognized as a nurse practitioner in a State, possess a master's degree from a program accredited by the NLNAC or CCNE, and maintain full and current certification as a nurse practitioner from the American Nurses Association or another nationally recognized certifying body.
Preferred Experience: Home Health Care experience preferred.
