The Advanced Practice Nurse provides primary care to complex, chronically ill patients who are commonly geriatricians, have palliative care needs, and have difficulty accessing primary care in traditional methods. The patients receive primary care in their own home; the service area is a 100 mile radius from the Fort Harrison. Primary care includes assessment, diagnosis, and treatment of healthcare problems. This includes initiating and adjusting therapeutic regimens, performing histories and physicals, ordering diagnostic and therapeutic tests and studies, initiating consults and referrals, and prescribing medications and supplies in accordance with VA policy, VA formulary, State regulations and scope of practice. He/she is responsible to contribute to the Veterans' plans of care. S/he is responsible to coordinate care in a manner that is conducive to producing optimal healthcare outcomes, quality, and safety.
The Advanced Practice Nurse, as a primary care provider collaborates with other members of the Home Based Primary Care interdisciplinary team in providing quality care and patient education. This Advanced Practice Nurse serves on medical center, VISN, and/or national committees and assists in the development, evaluation, and communication of policies and standards of advanced nursing practice.
The Advanced Practice Nurse also serves as the Case Manager coordinating the interdisciplinary care provided by the team. The Advanced Practice Nurse is responsible for the coordination, development and implementation of the Veterans plan of care in collaboration with the Veteran and members of the interdisciplinary care team. S/he collaborates with community providers to support the Veterans ability to reside safely and successfully in the desired community setting. S/he assists the Veteran with discharge plans to facilitate continuity of care across the continuum of care.
Work Schedule: Monday through Friday; 8:00 to 4:30 or as agreed upon with the possibility of extended hours and weekends. NOTE: Candidates must be willing to work weekends, irregular work hours and extended hours. The candidate must be willing to be assigned to other related services and/or locations, if necessary.

VA Montana has a RN Residency/Transition to Practice Program. Per VA Directive 2011-039 participation in this program is required for all nurses who have less than 1 year of registered nurse experience or as recommended by the Associate Director Patient Care Services.
Preferred Experience: Experience in Case Management as well as 2 years of experience as a nurse practitioner.
Experience refers to paid and unpaid experience, including volunteer work done through National Service programs (e.g., Peace Corps, AmeriCorps) and other organizations (e.g., professional; philanthropic; religions; spiritual; community; student; social). Volunteer work helps build critical competencies, knowledge, and skills and can provide valuable training and experience that translates directly to paid employment.
