Duties:
VA Medical Center, Canandaigua, NY and Rochester Outpatient Clinic, Rochester, NY are accepting open continuous applications for Primary Care Physicians.
The PCP is a core team member in the PACT model and is responsible for leading and collaborating with their team of health care professionals to provide comprehensive primary care in collaboration and partnership with the Veteran. The PCP evaluates, provides diagnoses and provision of care to adult patients for the treatment of general medical problems and makes referrals to specialty care to meet the needs of the Veteran. The PCP addresses a large majority of personal health care needs and develops a collaboration and partnership with Veterans. The responsibilities of the PCP include but are not limited to: diagnosis and management of acute and chronic medical and psychosocial conditions, health promotion, disease prevention, overall care management, post deployment care, Veteran/caregiver education and potential tele-health primary care.
Board Certified or Board Eligible in Internal Medicine preferred.
Work Schedule: Duty hours are Monday - Friday, 8:00 am to 4:30 pm. As a part of our elite team of providers, the incumbent will also be required to rotate through Saturday Primary Care Clinics and weekday evening hour clinics to provide extended care opportunities for our Nations Veterans.
