Major Duties:
The Philadelphia VA Medical Center (PVMAC) seeks board certified or board eligible internal medicine or family practice physicians to provide round the clock coverage to a 15 20 acute medicine patients and general /internal medicine consultation services. The incumbent functions as the Hospitalist on Duty (HOD) and is responsible for provision of care to a cohort of acute care patients for day or night coverage (8 or 12 hour shifts) including weekends and federal holidays. Shifts will generally be rotating 12 hours shifts 7 days a week (Monday-Sunday schedule). With approximately two weeks of paid sick and vacation time, taken into consideration, this equates to about thirteen 12 hours shifts or twenty 8 hour shifts a month.
The shifts will predominately start either 7:00AM or 7:00PM. However, start and end times and duration of each shift may vary. Number and pattern of days on or off may also vary. In the days, the acute medicine team includes two physicians and a physician assistant. At night a single physician covers the service. The HOD will be called on to perform the Medical Officer of the Day (MOD) function when the need arises to assist with patient flow from the emergency room.
The applicant should have experience in the management of medical conditions requiring hospitalization, be competent in performing routine hospital procedures such as lumbar punctures and central line placements, and be committed to in engaging in ongoing process improvement.
The Philadelphia VA Medical Center is closely affiliated with the University of Pennsylvania Medical School and there is an academic or teaching acute medicine service. However, this particular hospitalist position does not oversee or work closely with the medical residents or interns. Generally, the two services operate independently of each other, but the incumbent may need to work with or advise internal medicine house staff that will be caring for patients within the same wards and respond to all hospital emergencies. If there is an interest and qualifications, there could be rotations as a teaching attending, 10 to 20% of the time.
Job duties include but are not limited to:
- Daily rounds, physical examination, reviewing progress notes, labs, x-rays, and other chart data and writing progress notes during the day. Daytime or nighttime coverage would provide appropriate charting for any change of status of the patient including updates for change of medication, transfers to other units and new radiographic findings that change patient management during the "coverage" hours.
- Provider history and physicals or transfer acceptance notes and medical care plans to all patients assigned to the in-patient non-teaching service up to the capacity of the service.
- Provide inpatient medical consultation.
- Coordinate care with the nurse care coordinators, social work service, rehabilitation services, consultation services, interventional services and outpatient primary care to facilitate excellent quality of patient care, efficiency and good outpatient follow-up. All patients can be followed with-in the VA primary care so this enhances good patient care.
- Respond to all medical emergencies in the hospital while on duty and assist or run codes until the emergency response team is on site.
- Provide accurate and helpful sign-outs (or hand-offs) to the next team.
- Complete discharge summaries within 24 hours of discharge as well as communicating discharge to primary care provider.
- Participate in any on-going hospital initiatives as deemed appropriate such as quality improvement initiatives, clinical pathways etc.
- Oversight of physician extenders will be required, though the extenders work predominantly day shifts.
Work Schedule: Full-time- Weekday and Weekend Shifts to include Federal and Non- Federal Holidays.

Qualifications:
Citizenship: Must be a United States Citizen (non-citizens may be appointed only if it is not possible to recruit qualified citizens).
English-language proficiency: Must be proficient in spoken and written English.
Education: Degree of doctor of medicine or an equivalent degree resulting from a course of education in medicine or osteopathic medicine. The degree must have been obtained from one of the schools approved by the Secretary of Veterans Affairs the year in which the course of study was completed. Approved schools are:
(1) Schools of medicine holding regular institutional membership in the Association of American Colleges for the year the degree was granted; (2) Schools of osteopathic medicine approved by the American Osteopathic Association for the year which the degree was granted; (3) Schools (including foreign schools accepted by the licensing body of the State, Territory or Commonwealth or the District of Columbia as qualifying for full or unrestricted licensure.
Licensure and Registration: Current, full and unrestricted license to practice medicine or surgery in the state, territory or commonwealth of the United States or in the District of Columbia.
(1) Must meet basic qualification standards for appointment. Clinical privileges must be recommended by the chief, staff/professional standards board as granted by the director of the Philadelphia VA Medical Center. Incumbent must provide complete credentials and privileging information in a timely expeditious way. Board certification or board eligibility in Internal medicine or family practice is required. Incumbent must be competent in performing routine medical procedures such as lumbar punctures and placement of central lines.
PREFFERED EXPERIENCE:
1. Experience working in a teaching hospital.
2. Experience participating in and/or overseeing process and quality improvement teams.
