The incumbent will be located at the Portland Veterans Affairs Medical Center. The incumbent reports to the Supervisory Medical Instrument Technician (MIT), of the Sleep Disorders Center, through the Respiratory Care Manager along with the Sleep Lab Medical Director, Medical Director of Respiratory Care Services and the Clinical Director of Division of Hospital & Specialty Medicine (DHSM) . The Sleep Center Medical Director has staff authority of the overall functioning of the Sleep Disorders Center, and reports to the Chief of Pulmonary who reports to the Clinical Director of the Division of Hospital & Specialty Medicine who reports to the Chief of Staff.
Overnight Sleep Studies
Checks in and stages patients for overnight Sleep Study appointment and Titration study. Assists patients getting settled into the sleep lab for an overnight stay.
Receives/Interprets Requests
Receives requests and/or instructions for procedures based on the sleep physicians orders. Interprets requests or instructions from physician or medical officers. Receives assignments that require application of standard and non-standard procedures and seeks advice or the assistance of the Chief technologist and Clinical Director when questions or uncertainty about a procedure arises. Employee understands and carries out the requests made by physicians and when an uncommon, unique or new procedure is required consults with a physician for guidance. Receives assignments for a variety of difficult examinations on compromised patients for planned, urgent and emergent procedures. Communicates with personnel on inpatient wards and outpatient areas to obtain pertinent information in the planning of care during the procedure.
Secures Patients Confidence/Cooperation
Secures the confidence and cooperation of patients during procedures. Independently explains the procedures and positions the patients for a variety of procedures taking into account the patient's age, mental and physical status in order to alter the information delivery to the patient. Appropriately prepares patient for procedure using standardized aseptic technique. Performs triage, rearranges schedule according to patient priorities, oversees lower graded employees. Provides an assessment of the patient's condition throughout the patient's stay in the Department and responds appropriately when a patient's condition falls below the baseline assessment of what is considered to be stable.
Sets Up Equipment, Determines Technical Factors, Performs Quality Control Assessments, and Operates Equipment
Preforms any sleep study, whether it's a Diagnostic daytime study, a Split Study, a PAP Titration, a Maintenance of Wakefulness Test (MWT), or a Multiple Sleep Latency Test (MSL T) test. Prepares, calibrates, interfaces, and operates all equipment used in polysomnographic procedures. Applies electroencephalogram (EEG) electrodes based on the International 10-20 electrode placement system. Applies electro-oculogram (EOG), electromyogram (EMG) and electrocardiogram (ECG) using Rechtschaffen and Kales standardization manual. Applies pulse oximeter ear or finger probe; thoracic and abdominal effort belts; nasal/oral thermistor and thermocouple sensors to produce flow signals, and other diagnostic electrodes and equipment. Ensures patient comfort and proper function of applied electrodes and transducers. Monitors patient, diagnostic equipment, recording and video equipment throughout the study; obtains artifact-free data and tracings; record significant observations such as sleep stage, heart rate, respiration rate, oxygen saturation level, body position; presence and/or absence of sleep apnea, snoring, myoclonus, bruxism, cardiac arrhythmias, sleep terrors. Annotates seizures, abnormal sleep behavior and significant respiratory events on the electroencephalographic records in real time. Maintains proper equipment function and data collection. Responds appropriately to all situations involving patient care and maintains proper oxygen and CPAP settings.
