Assist with patient care, clinic workflow and assigned by NM or Charge Nurse to provide safe patient care. Uses the nursing process and evidence-based practice to collaborate with the Teamlet and larger Team to meet patient care needs.
Provides patient and family health education with a focus on self-management, prevention, and wellness, based on the patient's goals.
Demonstrates advanced clinical knowledge in assessing planning, implementing, documenting, and evaluating care for a designated group of patients.
Triages and applies a collaborative team approach in identifying, analyzing, and resolving patient care problems.
Collaborates with the interdisciplinary team.
Completes and documents Nursing Notes in CPRS, as applicable, in a timely manner and closes the Encounter Notes for patients not seen by a provider. Notes will be closed by the end of tour of duty daily. Areas to be addressed include: Encounters: reasons for visit-Service Connection; Primary assignment-Service connected conditions under the Diagnosis tab; Procedures, ACSC conditions, etc.
Monitors progress with clinical guidelines and/or pathways, analyzes variations and initiates appropriate actions.
Maintains current knowledge of multidisciplinary resources, programs and services, referring patients for community resources as appropriate.
Collaborates with patient and care providers.
Reviews options with patients and families, including costs, alternatives, risks/benefits and services.
Actively educates the patient and family on disease process, resources, plan of care, treatments and /or procedures.
Advocates fiscal responsibility in the management of patient care through effective utilization of resources.
Screens patients for social service, home care, and other community care needs; and coordinates or makes referrals as appropriate; and seeks consultation when indicated.
Appropriately documents own interventions and oversees appropriate health team documentation of patient care.
Keeps patient's provider aware of patient progress, issues, and/or problems.
Evaluates needs and facilitates the patient's ability to learn the principles of self-care; utilizes appropriate resources if the patient is unable to grasp the knowledge/skills needed for self-care.
Actively participates in care delivery with priority attention to patient preference.
Utilizes approved Protocols and Guidelines to facilitate autonomy in providing care.
Promotes cost effective use of resources; effective least-expensive strategies expected in non-face-to-face approaches (including telehealth)
Maintains privacy and confidentiality of all patient information, including electronic, print, and conversations.
Applies skills, such as motivational interviewing to promote patient self-management.
Promotes health promotion and risk reduction strategies
Actively participates in policies, procedures and standards to promote evidence-based patient-driven care.
Initiates follow-up telephone calls appropriately and documents interactions on encounter form/CPRS. Documents patient instructions and advice given.
Uses protocols appropriately and documents reasons for not following protocols.
Signs in and out of ACD system per policies/procedure.

The Associate Degree Nursing (ADN), Bachelor of Science degree in Nursing (BSN), or Master of Science degree in Nursing (MSN) must be fully accredited by the Accreditation Commission for Education in Nursing (ACEN) or the Commission On Collegiate Nursing Education (CCNE). VA TEXAS VALLEY COASTAL BEND HEALTH CARE SYSTEM
The VATVCBHCS is comprised of the Corpus Christi Outpatient Clinic, Corpus Christi Specialty Clinic, Harlingen Outpatient Clinic, Health Care Center at Harlingen, Laredo Outpatient Clinic, and McAllen Outpatient Clinic. The VATVCBHCS is an ambulatory health care system providing health care in medicine, surgery, psychiatry, and rehabilitation medicine. The VATVCBHCS also operates a 100-patient Home Based Primary Care program and a 200-patient Care Coordination Home Tele-Health Program. Through the utilization of inpatient/emergency department contracts, approximately 2600 inpatient admission and 5800 ER visits are managed by VATVCBHCS. Affiliated with the University of Texas Regional Academic Health Campus, the system has an active ambulatory care program and serves as a teaching and training site for medical residents. The VATVCBHCS is responsible for an enrollee population of over 30,000 veterans and 350,000 outpatient visits annually.
