Duties may include, but are not limited to:
-Coordinate patient care with other PACT team members, including nursing and clerical staff.
-Participate in daily huddles; participate in primary care systems redesign projects.
-Collaborate with primary care mid-level providers.
-Provide clinical oversight; participate in peer review of other primary care providers through periodic chart reviews.
-Act as surrogates for absent primary care providers when needed, to include review of CPRS view alerts.
-Regularly attend staff, departmental and general medical staff meetings.
-Document notes and encounters timely.
-Provide telehealth opportunity to Veteran via VA Video Connect and formal telehealth as needed by the department.
Work Schedule: Monday - Friday, 8am - 4:30pm
