VA Palo Alto Health Care system (VAPAHCS) is a large and complex VA health care system that provides veteran health care throughout a geographically large catchments area and is a major tertiary referral center. The Domiciliary Service at VAPAHCS provides supportive outpatient and/or residential rehabilitation to homeless and/or substance dependent Veterans with the ultimate goal of returning each individual to optimal independent living. In 1992 the Departments of Housing and Urban Development (HUD) and Veterans Affairs (VA) established the HUD/VA Supportive Housing (HUD/VASH) Program. The program is a collaborative effort, supported through HUD Section 8 rental assistance vouchers and VA's provision of intensive case management services. The primary goal of HUD/VASH is to move veterans and their families out of chronic homelessness. The primary component of the program is VA case management services, designed to improve veteran's health and mental health, enhancing their ability to remain stable, housed and community-integrated. The incumbent assists with program oversight, monitoring and administration, serving as a contact with local Public Housing Agencies (PHAs) and providing case management-based services (directly or through referral) necessary to ensure that veteran care is coordinated with the VA medical center, community health or mental health providers, and agencies that serve homeless populations.
The major duties and responsibilities include, but are not limited to:

mostly sedentary and requires walking, kneeling, bending, standing, and light
lifting.
BASIC REQUIREMENTS:
Preferred experience: At least two years outpatient mental health experience providing Housing oversight, working with Public Housing Agencies and providing case management services.
GRADE DETERMINATIONS:
GS-12 LEVEL: Experience, Education and Licensure. Senior Social Workers have experience that demonstrates possession of advanced practice skills and judgment. At least two years should be in an area of specialized social work practice. Senior social workers must be licensed or certified by a state at the advanced practice level which included an ASWB advanced generalist or clinical examination, unless they are grandfathered by the state in which they are licensed to practice at the advanced practice level (except for licenses issued in California, which administers its own clinical examination for advanced practice). Senior social workers may have certification or other post-masters training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship or equivalent supervised professional experience in a specialty. In addition, the candidate must demonstrate the professional KSAs below.
