The Intensive Community Mental Health Recovery Program (ICMHR) is part of the MT VA Mental Health Service Line. The ICMHR program provides clinical community-based case management services to Veterans with Serious Mental Illness (SMI) diagnoses, severe functional impairment, and high inpatient mental health unit utilization, in coordination with existing community and VA services.

Duties include but are not limited to"
- Completes screening and assessment of Veteran referrals, verifies a clinical need for program participation and presents information gathered to the ICMHR treatment team for admission consideration.
- Provides specialized treatment of complex mental illness and advanced level case management interventions in treating Veterans recovering from serious mental illness and co-occurring psychosocial, medical, and psychiatric conditions.
- Effectively functions as a member of an interdisciplinary team of Social Workers/Counselors, Nurses, Psychiatrists, Vocational Rehabilitation Specialists, etc.
- Provides intensive case management in the Veteran's home and in the community, focusing on rehabilitation, crisis/stress management, crisis resolution, problem solving, skill building and improvement in independent functioning.
- Employs a variety of psychological, rehabilitation and mental health techniques including evolving and evidence-based practices and provides individual, group, and family psychotherapy.
- Incorporates complex multiple causation in differential diagnosis and treatment of Veteran patients, including making psychosocial and psychiatric diagnoses. - Coordinates Veteran care and serves as an advocate on behalf of Veterans in the program.
- Provides Veterans and their family members with community outreach, counseling, therapy, direct services and referral to VA and community resources.
- Exercises judgment and decision making in the formulation of treatment plans in conjunction with the interdisciplinary treatment team and Veteran.
- Completes regular and ongoing assessments of the Veteran's progress and treatment and engages the Veteran in discharge planning as necessary.
- Empowers and respects the Veteran's right to self-determination while helping the Veteran identify and articulate their strengths, needs, abilities and preferences.
- Participates in weekly interdisciplinary team case conferencing meetings to review ongoing Veteran treatment, progress towards goals and Veteran crises.
- Follows all VA directives, handbooks, local policies, and SOP's.
- Provides ongoing documentation in the medical record of Veteran progress according to VA policies and documentation requirements for CARF and JC accreditation.
- Partners with VA and community agencies/stakeholders. Acts as a resource and liaison to ensure continuity of care. Collaborates to address Veteran needs or concerns.
- Facilitates psychotherapy and support groups related to recovery, skill development and community reintegration.
- Coordinates or participates in Veteran hospitalizations, discharges, transfers between levels of care, and participates in ongoing treatment team meetings, both outpatient and inpatient.
- Assists Veteran in identifying and applying for VA, Social Security, and other benefits.
- Drives government vehicles to Veteran homes and in the community or to transport Veterans as related to care plans. The incumbent must possess current driver's license and drive a government vehicle in carrying out professional duties when deemed necessary.
- Uses computer, cell phone and other technologies for the purposes of communication and documentation.
- Displays high standards of ethical conduct and takes an appropriate ethical course of action when alerted to concerns. Uses sound judgement and behaves in an honest, fair, and trustworthy manner. Conducts practice in accordance with professional ethical standards.
In carrying out responsibilities it will be necessary for the incumbent to travel into the community. The incumbent must possess a current driver's license and drive a government vehicle in carrying out professional duties. Incumbent will transport Veterans in GSA vehicles and must meet facility driving standards in accordance with established policy.
