Primary Care is seeking to fill a Primary Care Provider. This vacancy may be filled by a Nurse Practitioner through this announcement or by a Physician through announcement CBTD-11141186-21-CL.
The Nurse Practitioner assesses, diagnoses, plans, prescribes, intervenes, evaluates and delivers or participates in the delivery of health care to Veterans. This scope includes mechanisms of collaboration including record reviews, referrals, consultations and patient coverage. The Nurse Practitioner holds a current license to practice, is prepared for advanced nursing practice by virtue of knowledge and skills obtained through a post-basic or advanced education program of study acceptable to the State Board of Nurse Examiners. The Nurse Practitioner is prepared to practice in an expanded role to provide health care to individuals, families, and or groups in a variety of settings. The Nurse Practitioner acts in collaboration with other health care professionals to deliver health care services. The Nurse Practitioner is uniquely qualified to resolve unmet needs in primary health care by serving as an individual's point of first contact with the health care system. This contact provides a personalized, Patient-Centered comprehensive continuum of care and integrates all other aspects of health care over a period of time. The focus of care is on health surveillance (promotion and maintenance of wellness), but also provides for management of acute and chronic illness in order to maintain continuity.
