The Primary Care Clinic Manager ensures that the care provided at the Community Based Outpatient Clinic (CBOC) or medical center based primary care clinic meets the Joint Commission standards for outpatient care as well as the VA Performance Measures. The Clinic Manager is responsible for the documented outcomes at the program or service level. Program or service level outcomes must be broad and complex and can be demonstrated at any organizational level within a facility, VISN, or VACO. Program is defined as a substantial coordinated group of activities impacting patients and/or employees.
The Clinic Manager demonstrates
(a) Leading and organizing delivery of care to assure continuity of care and peer accountability for practice, including access to care and discharge planning.
(b) Guiding, developing, and supporting staff from a leadership perspective, and
(c) Using professional standards of care, scientific evidence, and practice to evaluate programs and/or service activities.
The Clinic Manager is the first line supervisor, registered nurses, licensed practical nurses, ancillary clinical staff, and clerical/administrative staff assigned to the Primary Care Clinics. He/she collaborates with the Associate Chief of Staff (ACOS), providers, Mental Health staff, medical and administrative leadership to facilitate optimal safe patient care and flow in Ambulatory Care and the Medical Center. The incumbent is also expected to participate on national and VISN teleconferences related to ambulatory care and to function as the Point of Contact for implementation of VHA mandates and directives related to PACT and outpatient care.
The Clinic Manager is accountable to the Associate Chief Nurse for Primary Care. The incumbent is responsible for supervising, organizing, directing, integrating and evaluating patient care delivery for Patient Aligned Care Teams.
Work Schedule: 8:00a.m. - 4:30p.m.
