The Clinical Dietitian is the medical nutrition expert responsible for planning, developing and directing all nutritional care activities for assigned clients. He/she supports the facility's mission and coordinates nutritional care with other health care providers, the client, the client's family and the community. The increasing complexity and specialization in nutrition requires that the clinical dietitian have expertise in a wide range of disease entities, treatment modalities and drug/nutrient interactions. The Telehealth MOVE! Dietitian has full responsibility to provide program support to the MOVE! Coordinator and other staff involved in providing care to the MOVE! Weight Management Program patient. Major responsibilities include but are not limited to: planning strategies to manage and deliver nutritional aspects of the MOVE! Weight Management Program; maintaining Telehealth nutrition care standards and providing education for the MOVE! Weight Management Program; providing quality assurance evaluations, professional development; improving the effectiveness, productivity, and satisfaction of patients; developing policy and guidance; maintaining continual professional development and developing clinical tools for use in the field.
- Nutritional Care: Conducts all aspects of the Nutrition Care Process (NCP) for assigned patients in accordance with hospital policies.
- Nutrition Education: Assesses needs and provides nutrition education for individuals and groups, including patients and their significant others.
- Quality Management/Improvement: Supports clinical nutrition performance improvement activities and participates in quality improvement projects in specialty area.
- Participation as a Member of the Health Care Team: Serves as a member of the interdisciplinary treatment team.
- Professional Development: Maintains dietetic registration and continuing education requirements.
- VHA Mission: The Telehealth MOVE! dietitian provides excellent comprehensive and innovative nutrition and food service programs for our Veterans and stakeholders that are evidence based and support the full continuum of healthcare through interdisciplinary collaboration both within and outside the James J. Peters VA Medical Center.
- VHA Vision: The Telehealth MOVE! dietitian will deliver premier level nutrition and food service programs to Veterans and stakeholders, using people centered, results driven and forward looking principles.
FUNCTIONS OR SCOPE OF ASSIGNED DUTIES:
A. NUTRITIONAL CARE
1. Develops and implements an individualized plan for medical nutrition therapy for MOVE! patients in accordance with the patient's medical goals and objectives. Accurately obtains and interprets medical information pertinent to the patient's nutritional care to determine risk, assess nutritional status and develop individual specialized care plans to simultaneously address several disease processes. Utilizes patient history and data collected from the MOVE! 11 questionnaires in order to assess nutritional, behavioral and physical activity needs of individual patients.
