The VA New Jersey Health Care System (VANJHCS) is one of five Center of Innovation sites in the VHA selected to transform the VHA's health delivery model into a more patient-centered, prevention-based model which supports the Whole Health and Wellness of the Veterans who we serve. The Center of Innovation/ Center for Health and Wellness integrate primary PACT, prevention, mental health, and complementary and alternative medical care services to meet the Whole Health needs of our Veterans. VANJHCS has been chosen to become a Whole Health Flagship demonstration site for VISN2.
Specific duties include:
-Capable of providing strong leadership.
-Apply, advanced professional nursing theories and techniques to the full range of patient populations.
-Provide professional advice and consultation in areas related to professional nursing and behavioral health management.
-Knowledge of clinical research literature.
-Effectively develop and implement health behavior interventions for the promotion of general health and to address health risk behaviors as part of disease prevention and chronic disease management (e.g., weight loss and management, tobacco use cessation, promoting patient self-management interventions targeting issues such as increasing participation in regular exercise, healthy eating, sleep hygiene).
-Ability to train, guide, mentor and support patients using web, home messaging, or video teleconferencing technologies, including supporting others in co-managing group intervention at parent facilities and CBOCs.
-Consult with and support the other primary care, prevention, and patient health education team members in conducting preventive medicine programs.
-Ability to effectively communicate with a wide range of healthcare staff and facility leadership to integrate health promotion and disease prevention programs with primary care.
-Comfortable in the role of administrator, educator, and direct services provider, as the needs of the program require.
-Familiarity with statistical reporting and program evaluation.
-Serves as a facility content expert for HPDP and oversees the integration of HPDP services into patient care throughout the facility and affiliated community-based outpatient clinics (CBOCs.)
-Knowledgeable about HPDP, as well as all outreach and local community resources.
Work Schedule: Monday - Friday; Full time; 8:00am - 4:30pm.
