    Summary

    The Caregiver Support Program Nurse reports to the Caregiver Support Program Manager and supports provision of services for Veterans and caregivers in the Program of Comprehensive Assistance for Family Caregivers and those applying to the program.

    Responsibilities

    The incumbent will function in support of caregivers, Veterans and family members throughout the medical center and in community settings. The incumbent will conduct initial and annual home visits or in-home wellness contacts to Family Caregivers in the program. The incumbent may conduct the Veteran Functional Assessment Instrument (VFAI) and/or other assessments needed via phone and clinical video telehealth. The incumbent is responsible for developing and establishing linkages to collaborate on VA caregiver support efforts with community healthcare and social service agencies. The incumbent possesses advanced knowledge and understanding of the services and benefits available for caregivers from both federal, state and private sector resources. Other duties may include conducting initial assessments with Veterans and caregivers applying to the program

    Pay schedule: NF23

    Work Schedule: Flexible - Supervisor will allow compressed tours
