This position is located within the Acute Care Department at the Pine Ridge Service Unit, Pine Ridge, SD. The purpose of this position is to provide case management services needing coordination of care, promotes the efficient and effective use of variety of health care resources by focusing on the whole patient to provide seamless continuum of care, and serves as an advocate for the patient and their families.
- Applies case management essential activities assessment, planning, implementation, coordination, monitoring, evaluation, out comes across the continuum of care.
- Maintains current knowledge of resources available within the community, maintain supply of resource material to be distributed to patients when needed.
- Makes rounds of patient care areas to review charts for appropriateness, completeness, and timeline of documentation by providers.
- Makes daily rounds to meet newly admitted patients for assessment/referral of patient's clinical needs.
Occasional travel - You may be expected to travel for this position.
